Job Summary

Develops and implements
programs and interventions related to the social determinants of health.
Supports CountyCare by ensuring integration of community and social services to
achieve improved population health outcomes and cost-effective use of health
care. Works independently and collaborates with diverse community stakeholders.

This position is exempt from Career
Service under the CCHHS Personnel

Typical
Duties

  • Drafts program description, work plans, annual
    program goals for social services, for approval by CountyCare leadership and
    oversight committees
  • Ensures that plans and program goals reflect the
    needs of CountyCare membership and quality of care goals and strategic
    directions of CountyCare and CCHHS, including NCQA health plan accreditation
  • Recommends, organizes and manages processes to
    comply with regulatory requirements, vendor agreement and best practices
  • Recommends and oversees social services programs and
    resources as part of CountyCare’s overall service package
  • Serves as a subject matter expert on community and social
    services and impacts on individual and population health
  • Performs environmental scans and needs assessments
    including but not limited to housing, crisis management, income support,
    food/nutrition, legal support, peer/consumer advocacy support, community
    programs, outreach/engagement and training and development
  • Makes recommendations based on assessment findings
    considering member demographic, geographic and risk stratification variables to
    recommend priorities for program development
  • Prepares and monitors program budgets
  • Monitors performance targets
  • Draft reports for various audiences
  • Collaborates and coordinates people and organizations to
    leverage CountyCare and CCHHS resources
  • Fosters strong relationships with diverse stakeholders
    from all community sectors
  • Engages contracted/delegated
    care management entities and medical management staff, vendors, behavioral and
    physical health providers to integrate social service interventions
  • Facilitates
    and promotes timely problem identification, analysis and resolution
  • Assists individuals who contact CountyCare
    as needed with immediate needs and linkage to ongoing services
  • Travels to work sites throughout Cook County
    including community agencies, network providers, government and other agency
    offices

Minimum Qualifications
  • Bachelor’s
    or higher level degree is required.
  • Three (3) years of total experience in social
    and/or community services
  • Program
    management experience is required.
  • Must be
    able to travel to work sites throughout Cook County including community
    agencies, network providers, government and other agency offices. Travel
    throughout Cook County is required.

Preferred Qualifications
  • Master’s
    degree OR other degree as a Doctor of Nursing Practice (DNP),Nursing
    Doctorate (ND), Doctor of Philosophy
    (PhD), Doctor of Psychology (PsyD), Juris Doctor (JD), Medical Doctor (MD),
    Doctor of Osteopathy (DO) or Doctor of Public Health (DPH) is preferred.
  • Project management
    certification is preferred.
  • Experience within a
    Medicaid Managed Care Organization is
    preferred.
  • Licensure as a Social
    Worker, Case Manager or Counselor in State of Illinois is preferred.
  • Bilingual
    in English/Spanish is preferred.

  • Knowledge
    of Cook County community services, resources, needs and current social services
    issues affecting Cook County resident and Medicaid recipients
  • Knowledge of trends in behavioral healthcare,
    managed care, Patient Centered Medical Home implementation, Medicaid, case
    management, medical management and quality improvement
  • Excellent
    verbal and written communication skills necessary to communicate with all
    levels of staff and a
    patient population composed of diverse cultures and age groups
  • Strong
    interpersonal skills to build relationships with employees at all levels of the
    organization/partners organizations including negotiation, team building and
    project management
  • Demonstrated attention to detail and creativity at the programmatic
    level
  • Data collection and evaluation skills
  • Ability to
    function autonomously and as a team member in a multidisciplinary team
