What You Will Do:



University Maryland Medical System is currently Seeking a Nurse Navigator (LPN required) at our Midtown campus.

Job Summary:
Under direct supervision by a Registered Nurse, provide overall coordination, support and expertise through timely assessment, planning and implementation and evaluation of individual patient needs and readiness for discharge. The overall goal is to enhance quality of care, promote continuity of care and efficiency through the integration of the discharge process.

Principal Responsibilities and Tasks

Patient Education Duties:
  • Educate the patient about his/her disease, disease process and treatment plan
  • Reason for hospitalization.
  • Review/Discuss all procedures and tests ordered by the physician and its importance.
  • Review medications that are ordered and their indications.
  • Discuss/review laboratory, radiology and other needed procedures/tests.
  • Discuss and coordinate all patient care needs with the health care team and assist in resolving potential issues prior to discharge.
  • Instructions on what to do if condition changes after discharge.
  • Ensure patient and or significant other understand the plan of care post discharge.
  • Coordination Duties:
  • Ensure appointments are coordinated timely with patient input regarding the best time and date of the appointment.
  • Coordination for follow up tests and studies.
  • Coordinate appointments with physicians, testing and other services.
  • Discuss importance of physician appointment and follow up.
  • Ensure patient knows where to go, assist resolve transportation issues and keeping appointments.
  • Coordinate needed information for the patient from the nurses, social workers, physical therapists and physicians that will enhance discharge efficiency.
  • Administrative Duties:
  • When out in the community review the written discharge plan making sure patient understands:
  • Reason for hospitalization
  • Prescribed medications which include what to take, when to take them and how to obtain the medication.
  • Written instruction on diet, activity and “do’s and don’ts if any.
  • Ensure that ordered home care equipment are delivered to home.
  • Obtain consent for the transitions program, doing a pre-assessment of the patient prior to discharge.




  • What You Need to Be Successful:



  • Licensed Practical Nurse (LPN), valid and current license by the Maryland Board of Nursing.
  • Working knowledge, and/or related experience in housing, social service and individual support services, Medicare/ Medicaid services, or home and community based services.
