Nurses Make It Possible

A career with NewYork-Presbyterian’s Ambulatory Care Network (ACN) is dynamic and often unpredictable, bringing new challenges to each and every day. With a variety of hospital based and community based settings and several subspecialties, including Psychiatry, Internal Medicine, Pediatrics and Surgery, there are several paths for success—and countless opportunities to impact the communities we serve. The primary care sites are all designated as NCQA Patient Centered Medical Homes.

The ACN provides high-quality care to a diverse group of patients, regardless of socioeconomic status. Our leadership takes great pride in being able to stay involved in every stage of care, from the development of best practices to the coordination of multidisciplinary services. And through our Community Outreach Program’s various initiatives and events, ACN leaders can help safeguard and improve the health of at-risk children and adults in the New York metropolitan area. The ACN is also participating in several strategically important hospital initiatives including care management, readmission reductions, and Medicaid redesign programs.
