Job Summary

Under the general supervision of the CEO and/or the CFO/COO, the Nurse Manager organizes and directs the work of the nursing staff and the day-to-day delivery of nursing services to patients in the community; he/she coordinates staff to assure effective clinical care and quality standards, supervises the direct-care nursing staff and coordinates the patient care team

Job Responsibilities and Functions

The function of the Nurse manager is to develop and maintain an environment that supports the professional nursing staff. The Nurse Manager coordinates, supervises and manages the day-to-day activities related to the delivery of quality care nursing services to a primarily geriatric adult population in the Metro-West community. Responsibilities include interdisciplinary patient care coordination and supervision to ensure compliance with practice and regulatory guidelines; clinical supervision and development of direct nursing and social service staff to promote quality care and positive outcomes; and documentation review to validate care delivery and compliance with pertinent standards and regulations.

Qualifications

  1. Registered Nurse, with a current license in good standing to practice in Massachusetts
  2. Baccalaureate Degree or a Masters Degree in Nursing from an accredited school of nursing
  3. Minimum of three years clinical experience with at least one-two years in a Medicare certified home health agency
  4. Current knowledge of Medicare Conditions of Participation and Massachusetts nursing practice guidelines
  5. Previous management experience preferred
  6. Certification in OASIS-C (COS-C or HCS-O) preferred

Leadership and Practice Responsibilities

  • Utilizes the American Nurse Associations Scope and Standards of Practice and the Code of Ethics for Nurses with Interpretive Statements, as the foundation of agency nursing practice
  • Promotes agency mission, values and philosophy to ensure quality patient care
  • Utilizes evidence-based practice to develop and implement standards of practice to guide clinical improvement initiatives.
  • Enforces and follows agency polices, practices and guidelines as per the Employee Handbook and the Natick VNA policy & procedure manual and Performance Improvement Plan
  • Provides administrative and professional leadership, direction and guidance to clinical staff
  • Participates in recruitment and retention activities and processes as directed: interviews, recommends for hire, motivates

Operational and Care Coordination Functions

  • Identifies patient care priorities and assigns staff accordingly
  • Coordinates clinical case load assignments and productivity expectations
  • Coordinates care on the weekends including, but not limited to, referral entry and follow-up, monitoring of telehealth patients
    Performs nursing visits when necessary
  • Conducts staff supervisions to ensure clinical competency of nursing staff as required
  • Holds nursing staff accountable for all aspects of day-to-day nursing care of patients, including required clinical documentation, adherence to Medicare Conditions of Participation.
  • Facilitates accurate, timely and efficient documentation of reimbursed services
  • Facilitates the development of skills, certifications and proficiency of clinical staff
  • Facilitates new staff orientation and related competency evaluation processes
  • Performs nursing visits when necessary

Performance Improvement Responsibilities and Functions

  • Promotes customer service by responding to and resolving difficult and sensitive patient and staff inquiries and complaints
  • Continuously monitors and evaluates the efficiency and efficacy of service delivery methods and processes, assesses and monitors workload, support systems and identifies opportunities for improvement
  • Works closely with the agencys QA/QI/Staff Development specialists, to ensure accurate, thorough, timely completion of OASIS assessments, MD order submission, patient care plan development, and all other required clinical documentation

Personal and Professional Development

  • Maintains professional practice standards and complies with all Massachusetts Board of Nursing licensing requirements
  • Provides first line support to the executive management team
  • Pursues opportunities to promote personal, professional and nursing leader development and educational advancement
  • Maintains proficiencies necessary to perform role and functions
  • Attends clinical and leadership educational events that pertain to home health nursing and the population served
  • Actively participates in home health industry initiatives and activities

Minimum Physical Requirements

  • Standing, sitting walking, bending, squatting, repetitive movements, driving
  • Weight lifting up to 50 pounds and weight transferring up to 150 pounds
