Home Care Liaison
|LOCATIONThis position will work out of the North hospital and support Community North, Community Heart and Vascular, and Community Rehab Hospital.
The home health liaison partners with network case management or case management designees and with selected non-network facility staff to evaluate patients’ eligibility for the provision of home care with Community Home Health Services (CHHS). The liaison will determine the patient and family needs and appropriate levels of care for patients referred to CHHS. The home health liaison will provide recommendations and support to facility case managers and following the patient’s referral to CHHS, will initiate home care discharge planning. The home health liaison will establish and develop ongoing proactive relationships with referral sources.
At least two years of hospital nursing experience.
ASN required, BSN preferred.
IN state license as a Registered Nurse
Assessment skills. Knowledge of Medicare and Medicaid regulations preferred. Strong interpersonal and communications skills required. Excellent organizational skills.
