CTT is mandated to provide services to those individuals within the mental health system who present the greatest degree of severity of symptomatology as evidenced by their multiple physical, psychiatric, and legal conditions, extensive use of services, and lack of follow-through with treatment. This population requires the highest level of medical necessity as defined by the state Adult Environmental Matrix.
As a member of the Blended Enhanced team, the CTT nurse provides an array of health care support to consumers, other team members and service providers. Utilizing the Community Support Program model, the CTT Nurse routinely assessed the psychiatric and physical health care needs of these persons. The incumbent developed a strength-oriented treatment/service plan that addresses consumer health care needs through the identification and utilization of community resources. The nurse is responsible to ensure that the mission, goals and philosophy of CTT are operationalized within the team, especially as they apply to health care services.
The incumbent works with staff to ensure continuity of care, and enhancement of participant’s understanding, acquisition and maintenance of independence living skills in the area of: (a) their daily living situation; ( b) interpersonal skills and social support/network building; (c) leisure and recreational support/skills development; (d) maintained and enhancement of physical and mental needs.
Duties and Responsibilities:
Coordinates the health care needs of assigned participants on two (2) BE teams and provides support to other team participants as needed during periods of vacancy:
Maintains records of medical history and ongoing medical records; completes annual and updated Nursing Health Assessments for CTT participants; ensures that discharge summaries/discharge sheets and medication protocols are available for all individuals hospitalized.
Consult with Team regarding development and implications of Nursing Service Plans, develop implements and modify Nursing Service Plans and will communicate health information to consumers.
Negotiate the health care responsibilities of each participant with provider agencies and CTT and facilitate the use of generic and community services.
Coordination with provider agencies, team members and consumers’ routine medical, vision, lab, and dental appointments. Use protocols for reporting of results and follow-up.
Accompany participants to an appointment when it is determined necessary by the nurse, and the team.
Attend psychiatric team meetings to review and update medications, Personal Plans and response to treatment. Attend team meetings to review the health care needs, participant’s response to treatment and coordinate the scheduling of appointments.
Monitors and administers participant medications (IM and oral).Coordinates, administers injections as needed; maintains and posts IM injections schedule in designed area.
Participates in discharge planning for participants from acute or extended care facilities.
Monitors and, as necessary, provides medical treatment in the community as prescribed by a recognized medical service vendor.
Complete Appropriate Documentation.
Develops, reviews, and revises forms, procedures, and standards of care according to accepted National Standards of Nursing Practice Act.
Completes Service Documentation as required for all participant related activities (billable and non-billable).
Serves as a member of the CTT Quality Improvement Program; conducts QA reviews of personal records.
Maintains nurses' section(s) of the participant Personal Records: updates FACE sheet medication list at least quarterly, routine lab work data, IM injections schedule, and acute hospital status as needed.
Provides consultation services as necessary and required to community agencies upon request.
Provides medical input to teams at daily team meetings, otherwise as requested; provides community services upon request in consultation with the Clinical Director.
Provides Educational and Training
Educational and training sessions are conducted with participants, families, provider agencies and CTT Staff. Subject matter and prescription is based upon individual and group needs. Training to meet accrediting body standards is also provided.
Performs others duties as required.
Skills Required: Must have excellent verbal and written communication skills. Ability to work well with people, have creative problem solving skills. Present current clinical skills; knowledge of behavioral health system/community resources networking skills; ability to work independently, flexible and adaptive in handling changing priorities in a fast paced work environment and able to work extended hours as necessary.
Educational and Experience: Bachelor’s Degree, Associate's Degree or a Diploma from an approved nursing program, with two (2) years of experience preferred working with persons with mental illness; experience may include internships and/or practicum. Must have Pennsylvania RN License in good standing.
Other Job Requirements: Must have a valid driver's license, auto insurance and vehicle for job related travel in the community; ability to work in a team environment and share office space; ability to work evenings and weekends in meeting on call responsibilities; able to work in the community a significant part of every work day.
Essential Functions: Ability to sit for one hour at a time, to use a telephone, to climb stairs at residences, bend, stoop, push and pull file drawers, to lift up to 20 lbs. and to work in community some part of every work day.
