Job Description

Dunson & Associates seeks R.N. Case Managers to work with complex needs for disease management and patient education. The Case Manager will share patients disease management goals, progress and other pertinent information with the healthcare team to ensure consistency of patient goals, facilitate tracking of patients progress and ensure quality outcomes.

Job Description

Access patient needs and determine priorities for disease management programs and services.

Conduct disease management assessments using standardized instruments and documents in EMR and/or care management platform as appropriate.

Act as a resource person for patient, provider, care center staff and the community.

Develop repository of best disease management practices for the disease management team and facilitate educational opportunities for patients.

Knowledge of Medicare and Medicaid reporting requirements preferred.

Documents patient care plan tasks, goals, and interventionstact

Perform outbound calls to patients approved by the multi-disciplinary team

Document significant specialty and diagnostic services and outcomes in patient care plan.

Identify caregiver training needs and tracks impact of needs and or training

Assist with discharge planning and coordination to ensure all post-discharge services required are in place and patient has scheduled post-discharge appointments

Identifies the appropriate utilization of resources across the continuum of care

Maintains patient - caregiver care plan compliance reporting. Submits reports to interdisciplinary care team weekly.

Participate in quality improvement and evaluation processes

Coordinate and direct the retrieval of diagnostic results from specialty referrals back to the clinic

Works closely with the pharmacy benefits and community pharmacies to help coordinate medication accessibility and medication refills.

Requirements:

R.N. preferably a bachelor's degree in nursing

Current RN License in good standing in the state of Washington

3+ years experience in an acute care setting as a health care professional in a clinical area

Knowledge of Medicare and Medicaid reporting requirements preferred

Experience with patients with chronic medical conditions ie congestive heart failure, coronary artery disease, diabetes, COPD or Asthma preferred

Demonstrates ability to perform multiple concurrent tasks with minimal supervision while meeting deadlines

Strong communications skills, oral, written and electronic

Working knowledge of Microsoft Office.

Prior UM experience a plus

Demonstrated passion for extraordinary patient service and personality for developing close relationships with patients.
