During life's most difficult moment you're the nurse they get to know and love. Homestead Hospice is seeking an amazingly compassionate Nurse Case Manager to join our team. The Nurse Case Manager performs routine visits Monday - Friday and meets with the family to understand all of their needs. Using your hospice experience you develop an ongoing plan of care ensuring all services needed are identified and met. This is not just a job and it requires a strong Hospice Heart. The nurse we are looking for doesn't just visit patients. They educate the family and caregivers with a warm and compassionate voice. The ideal candidate will have the ability to bond with the family and caregivers. The Nurse Case Manager walks with the family during this difficult time and coordinates all services with the Interdisciplinary Team.

Required Skills

RN Case Manager candidates must adhere to Homestead Hospice policies which include managing the changing needs of the patient and family. Your goal it to ensure their comfort and understanding during this difficult time.

Additional responsibilities include:

  • Initial visits for patient evaluation
  • Develop and adjust the plan of care as necessary to meet the needs of patient
  • Communicate assessment of potential patients and report any problems to Clinical Manager, Medical Director, and Interdisciplinary Team
  • Build relationships with all referral resources
  • Ensure ongoing eligibility of the hospice benefit

Required Experience

  • Associates of Science degree in Nursing
  • Hospice experience strongly preferred
  • A valid registered nursing license on file with the State of Georgia
  • A valid drivers license with clean driving record
  • Reliable transportation and ability to drive 30-60 miles per day
