RN Case Manager
Pathway Hospice in Columbia, SC is seeking a (PRN) RN Case Manager who will provide planning, direction, and coordination of patient cases within a hospice care environment. Case Manager will communicate with patients, families, and members of interdisciplinary team. Will case manager and assess, initially and ongoing, the impact of the terminal diagnosis of the patient's physical, functional, psychosocial and environmental needs and activities of daily living. Will develop or implement and evaluate the individual and plan of care with the patient/family/caregiver and other members of the interdisciplinary team. Provide management of discomfort and apply specialized nursing skills related to palliative and end of life care. Determine the scope and frequency of services needed based on acuity and patient/family needs. Assures quality care by reviewing and assessing each service and oversees the certified nursing assistant.
Must have a Registered Nurse license in South Carolina.
Must have previous hospice or home health nursing experience.Company Description
