Registered Nurse (RN) Case Manager (Training provided)
Large medical group in Southern California is seeking an Inpatient RN Case Manager in the Northridge area. Duties and Responsibilities:
An ideal candidate will be an individual who demonstrates clinical competency in properly assessing member needs and taking appropriate course of action to sustain quality care. This individual must be comfortable in assisting and assessing members in a telephonic capacity. In this telephonic role, the Case Manager is to conduct follow-up phone screenings with post-dispatch members. This is to minimize re-admissions back into the acute care setting while attempting to transition back into their daily lives. Candidates must be proficient at maintaining excellent communication with medical group members and colleagues alike.
- Current California RN license
- Two or more years of experience in an acute care, clinical setting
- One or more years of HMO experience
- BSN from an accredited institution highly preferred
