Staff Nurse

The ADHC Program Nurse is a registered professional nurse who is accountable to the Nurse Manager. In collaboration with the Nurse Manager, assumes responsibility and accountability for the day to day clinical operation and patient care at the Adult Day Health Care Program. This includes the management, supervision and evaluation of care delivered by nursing staff. The RN creates a positive work environment which enhances care delivery and professional development of clinically competent staff. The ADHC Program Nurse promotes, creates, demonstrates and ensures a culturally diverse workforce in which the values and needs of all individuals are respected. The RN is a core member of the ADHC Geri Pact and will work directly with the physician to provide primary care to the Veterans who attend the VA ADHC program

General Description of Assigned Duties:

  • Applies the nursing process to systems or processes at the unit/team level to improve care.
  • Skillfully performs in rapidly changing, atypical, or crisis situations.
  • Manages work place issues with consistency and documents appropriately.
  • Comprehensive health assessment and ongoing monitoring of physical status of the patient, preventive health care, health counseling and education for veterans and caregivers.
  • The ADHC staff nurse functions as the PACT nurse for veterans who receive their primary care from the ADHC Physician.
  • Will prepare for, and lead the patient care conferences, and maintain communication with the families/caregivers.
  • Establishes and communicates workplace priorities and makes staff assignments accordingly.

    Medication reminders for Veterans who take medications while attending the ADHC program. May provide set-up for home medication administration.

  • Provide care for urgent/emergent health conditions that arise while veterans are attending ADHC. Communicate this information to health care providers and families, and document appropriately in CPRS.
  • The ADHC staff nurse works with the other members of the ADHC staff, to provide hands on care to the participants. This will include activities such as: monitoring the door at arrival and departure time, covering the kitchen for coffee and lunchtime, serving and supervising the noon meal, toileting and ambulating veterans, and other duties to support the daily operation of the ADHC program.
  • Monitor and order program supplies and oxygen.
  • Assist the Nurse Manager to screening new participants.
  • Perform mini-cog assessments.
  • Venipuncture, lab draws, monitoring lab results.
  • Completes clinical reminders including; vaccinations, depression screening, TB testing, tobacco use screening, etc.
  • Skin assessments and foot care.
  • In depth knowledge of the management of patients with dementia.
  • Ability to promote team work and build positive relationships.
  • Knowledge of Performance Improvement, and participate in program initiatives.
  • The ability to mediate conflict and facilitate positive solutions, in coordination with and in the absence of the Nurse Manager.
  • Cleans RME (reusable medical equipment) per SOP.
  • Duties as otherwise assigned including assisting with community ADHC initiatives.
  • BASIC REQUIREMENTS:
    • U.S. citizenship. Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
    • Student of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The National League for Nursing Accrediting Commission (NLNAC) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6)
    • Have a minimum GPA (cumulative grade point average) of 3.5 on a 4.0 scale
    • Must pass pre-employment medical examination
    • Background and/or security investigation required
    • No service obligation to any federal, state, or private institution
    • Will be able to complete a minimum of 400 hours as a nursing VALOR student

    Education/Experience Requirements: Students are required to have had 2 semesters of clinical trainingassociated with their academic program.

    Preferred Experience: Community service

    Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religions; spiritual; community; student; social). Volunteer work helps build critical competencies, knowledge, and skills and can provide valuable training and experience that translates directly to paid employment. You will receive credit for all qualifying experience, including volunteer experience.
