Position Summary:

Provides patient care as part of a multi-disciplinary team based on national standards, and utilizing the nursing process. (Position requires a MINIMUM of one year experience.)

Position Expectations & Responsibilities:
Expected to demonstrate a good knowledge of regulations for patient care and documentation. Expected to have the ability to read, analyze and interpret general business periodicals, professional journals, technical procedures or governmental regulations. Expected to have basic knowledge of the Horizon Homecare system and be competent in the appropriate use and functioning of equipment in the area of clinical practice. Expected to have intermediate mathematical skills. Expected to demonstrate a good working knowledge of CMS/AHCA/OSHA/The Joint Commission regulations for patient care and documentation. Expected to deliver skilled, professional and age-appropriate intermittent patient visits at home. Expected to develop, prepare and maintain individualized patient care progress records. Expected to evaluate records of referred patients at skilled facilities and promote smooth transition to home care. Expected to assess the patient's continued care needs. Expected to reassess patient at each visit and revise plan of care as appropriate. Expected to recommend solutions for complex problems. Expected to coordinate the total plan of care as directed by physician and case conferences to assure continuity of care, and report changes as needed. Expected to participate in PI processes. Expected to complete all necessary documentation, department logs and reports. Expected to initiate and promote a multidisciplinary approach to patient care. Expected to demonstrate a commitment to personal, professional and clinical education as demonstrated through continuing education programs, participation in committees, maintaining competencies and review of current professional literature. Expected to recognize the influence of beliefs and cultures on behaviors and accept strengths and limitations in others. Expected to exhibit adaptability, flexibility, self control and maturity in work and behavior. Expected to exhibit calm responsiveness to emergency or stressful situations. Expected to recognize situations which require notification of a supervisor. Expected to complete assignments with promptness and accuracy. Expected to meet deadlines for all tasks assigned. Expected to prioritize work assignments and complete tasks in a timely fashion. Expected to be an active contributor in all customer service initiatives. Expected to be proactive in providing hospitality to all BRRH's patients, families, visitors, employees and physicians. Expected to serve as an ambassador for BRRH.
Field RN
Expected to submit accurate documentation within 24-48 hours of visit. Expected to teach, supervise and counsel the patient and family and/or significant other regarding home nursing procedures and care needs as appropriate for the patient. Expected to involve patient/family/significant other in total plan of care. Expected to demonstrate leadership skills through appropriate decision making, delegation and supervision of duties, and functioning as a preceptor/resource to new staff, and students. Expected to communicate plan of care changes to appropriate personnel. Expected to write reports, business correspondence and procedure manuals. Expected to effectively present information and respond to questions from groups of managers, clients, customers and the general public. Expected to communicate verbally and in legible writing in the English language in a manner that can be understood. Expected to attends and participate in department staff meetings as well as participate in hospital meetings, forums and seminars. Expected to supervise and teach the home health aide.
Minimum Education and/or Experience:
Diploma from an accredited school of nursing or Associate's degree
A MINIMUM of One year of experience as a Registered Nurse is REQUIRED
BLS certification from the American Heart Association Current Florida Licensure as Registered Nurse
*Incumbent may be required to perform emergency duty before, during and/or beyond normal work hours or days in the event of an emergency, crisis situation or disaster (man-made or natural) including evacuation sites.

*The person in this position will work in a smoke-free location, and is expected to adhere to all smoking restrictions.
