GENERAL SUMMARY:

The RN Case Manager for Sinai Health System has duties that include: facilitating timely and cost efficient discharge planning, coordinating the care of patients and ensuring timely patient movement through the system.
The RN Case Manager will partner with physicians, social workers, and other members of the multidisciplinary team to communicate and carry out the plan of care. Throughout the patient stay, the RN Case Manager performs activities to validate the medical necessity of the patient and interacts with insurers to discuss patient status, plans and continued stay requirements.

EDUCATION:

Graduate of an accredited school of nursing.
BSN Degree Preferred

LICENSE / CERTIFICATION:

CPR
Currently licensed as a Registered Professional Nurse in the State of Illinois.
MINIMUM WORK EXPERIENCE:
1-2 years experience in case management or utilization review
Knowledge of Interqual Acute Care Criteria and/or M&R Guidelines
2-3 years general nursing experience in a hospital setting
