Job Summary

The Clinic RN has the responsibility for providing direct and indirect nursing care to assigned patients. Works under the direct and indirect supervision of the PACE Clinic Center Manager or designee, but is independent in the application of advanced nursing knowledge and skills. Manages complex clinical situations for which he/she is responsible. The Clinic RN for PACE will also have additional responsibilities related to start-up development, as further described below.

Position Responsibilities

Assessment: Collect patient data and complete required forms with appropriate responses according to department and industry standards; identify patients overt problems/needs and sets priorities. Identify problems requiring further referral and/or follow-up; observe and record latest diagnostic results; perform advanced nursing observations using critical thinking skills. Collaborate with Medical Data Management and Quality Assurance staff, as appropriate, to run clinical core reports to assess quality and utilization in the PACE clinic.

Planning Care: Develop a plan of care based on nursing process and which incorporates the plans of other disciplines and continuing care needs; include the patient/family in developing or revising the plan of care; and make referrals to interdisciplinary support services. Initial and periodic assessments (minimally every 6 months) to be completed prior to the scheduled team meetings. Communicate participant changes to team members.

Implementing/Managing Care: Ensure care being provided conforms to accepted nursing practice standards and organizational/department standards; provide correct treatments/ procedures and other care as prescribed and according to patient care standards. Demonstrate thorough understanding of age-related characteristics, symptoms, and needs of patients served; explain nursing/clinical procedures in appropriate way; provide participant and family education in coordination of 24 hour care delivery; identify emergency situations and independently initiate advanced therapy; act as primary coordinator of patient care with other health care personnel and evaluate patient care measures instituted. Work with primary care physician to provide patient care and to implement physician orders as necessary and as appropriate to nursing standards. Document all nursing treatments accurately and within departmental time requirements into patients charts. Understand and demonstrate respect for patient rights and utilize established mechanisms for management of ethical issues in patient care. Track utilization of medical supplies and provide input to Center Manager and Medical Director regarding the re-ordering of medical supplies.

Evaluation: Record patient care delivered as planned and any variation, with appropriate rationale. Make and record observations, as well as implement nursing measures related to impending or associated problems. Make assessments and evaluations of clinical work delegated to clinical staff.

Leadership: Using nursing processes, delegate nursing care to appropriate personnel; integrate cost effective measures into nursing practice; recognize unit problems and take responsibility for documentation; demonstrate active participation in QI processes; comply with organizational expectations to meet staffing demands based on patient care needs; comply with policies addressing safe working conditions; monitor unsafe working conditions and recognize inappropriate and/or ineffective patient care management, resolve issues/ problems; complete written reports; fulfill call for unit as assigned and adjust staffing for volume and acuity as necessary; provide oversight and training of clinical staff (LVN, CNA, Medical Assistants) and provide staff education on nursing topics, as necessary; provide input to Medical Director and Primary Care Physician (PCP) regarding clinic triage protocols and procedures.

Interdisciplinary Team: Participate as an active and cooperative member of the Interdisciplinary Team. Attend staff meetings and take part in participant care planning.

Program Development: Work with Clinic Manager, PACE Primary Care Physician and PACE Medical Director to establish and achieve quality and productivity standards for the PACE clinic and its staff; assist the Clinic Manager, PCP and PACE Medical Director to establish protocols and procedures related to clinic; consult and educate internal and external staff to improve overall compliance with regulations and standards related to the PACE clinic, including facility procedures (sharps disposal, biohazard waste handling, etc); provide input on the selection of medical equipment; provide input to PACE department in the development and ongoing improvement of the PACE electronic health record system

Other projects and duties as assigned.

Possess the Ability To:

Only act within the scope of his/her authority to practice.

Meet a standardized set of competencies for the specific position description established by Company PACE and approved by regulatory agencies before working independently.

Be medically cleared for communicable diseases and have all immunizations up-to-date before engaging in direct patient contact.

Experience & Education

Graduate of an accredited school of nursing.

Current RN license in good standing to practice in the state of California.

3+ years RN clinical experience with at least 1 year working with the frail or elderly population. Relevant experience must be within the last 3 years.

Other training as required by the department.

Knowledge of:

Clinical nursing treatment of geriatric patients and their acute and chronic conditions.

Basic computer skills and use of electronic mail.
