Due to our dynamic growth, our Intake Department has full-time RN Care Coordinator openings at our partner hospitals - Columbia St. Mary's Hospital - Ozaukee located in Mequon, WI AND Froedtert Hospital located in Milwaukee, WI.

Intake is the hub of our Agency, and we need a nurse with organizational and interpersonal communication skills.  The role includes assessing, developing care plans, and working with hospital case management in coordinating and admitting patients into hospice and home care services. This is your opportunity to create a positive experience for patients and families.

Qualifications include a degree from an Accredited School of Nursing (BSN preferred), a WI RN license, at least 3-5 years of related experience, and computer/typing proficiency. Hospice/Palliative Care experience required.
