Job Description

Position Name: RN - Registered Nurse - LVN

  • Educate patients and their care givers on their conditions and steps needed to ensure proper recovery .
  • Provide admissions , case management and follow up skilled nursing visits for home health patients
  • confer with physicians in developing the initial plan of treatment based o physicians orders and initial patient assessment
  • Render hands on care ,management and evaluation of the care plan and teaching of the patient in accordance with physician orders ,under supervision
  • revise plan in consultation with physician based on ongoing assessments and as required by policy regulations
  • Coordinate appropriate care , encompassing various health care personal such as (Occupational Therapist, MSW, Home Health Aides ,and external providers )
  • Report patient care, condition ,progress to patients physicians and supervising nurses on a continuous basis
  • Implement patient care plan in conjunction with patient and family to assist them in achieving optimal resolutions of needs and problems
  • Oversee the work of home health aides and provide written personnel care instructions that reflect current plan of care
  • Discharge patients after consulting with the physicians and supervising nurse completing needed clinical documentation
  • Participating in the Quality Assurance/Quality Improvement Process
  • Must Posses a strong ability to submit nursing notes in an accurately ,orderly and timely manner.
  • Adhering to all practice standards as they apply to patient care and Medicare

Required Skills/Education

  • Associates of Science degree in Nursing
  • Valid RN license in state employment
  • Valid Drivers license with clean driving record
  • Ability to drive 20-50 miles daily as needed
  • Ability to successfully complete required back ground check and drug screen
  • Excellent Charting and Nurses Note Documentation and Coding, experience with electronic medical record required. Kinnser Preferred
  • Bilingual a plus
  • Must Posses a very strong ability to be a self starter with good organization skills, full ownership of responsibilities, Very Compassionate,, Conscientious, Excellent Work Ethics
  • Woumb Care, IV injections, inclusive but not limited to, etc....
