Community Health Centers, (CHC) is a private, non-profit organization that provides healthcare services to insured, uninsured, under-insured, and under-served children and adults within Central Florida. We are a Federally Qualified Health Center (FQHC) organization that began its operations in 1972 in Apopka, Florida. We now provide care to nearly 60,000 patients each year in the Apopka, Bithlo, Forest City, Lake Ellenor, Leesburg, Meadow Woods, Pine Hills, South Lake, Tavares and Winter Garden communities!

We are currently seeking a Clinical Center Coordinator (LPN/RN Nursing Supervisor) to join our Apopka team!

Primary Responsibilities and Specific Duties:

  • Collaborates with the Center Director/Manager to identify objectives and strategies for performance improvement in care coordination, communications and management of patient care.
  • Responsible for participation of team members and providers on pre-visit planning, huddles, one-on-one communication, regular site meetings and email to communicate and address patient care and operational requirements.
  • Provide ongoing training to team members of the center including care teams and operations teams.
  • Demonstrates an understanding of patient confidentiality to protect the patient and the health center.
  • Ensures daily and weekly tasks are completed.
  • In the absence of the Center Manager/Center Director, may act as their designee when requested.
  • Will be the primary designee for any patient complaints with the goal of resolving the complaint or escalating to the Center Manager/Center Director when complaint cannot be resolved.
  • Ability to organize, communicate and implement organizational and Center goals and objectives.
  • Under the direction of the Center Manager/Center Director:

o Supervises staff and work flow processes.

o Enforces adherence to policies and procedures including Time, Attendance and Punctuality policy.

o Prepares employee evaluations, disciplinary actions and applicable competencies.

o Conducts employee coaching sessions and disciplinary action meetings.

o Prepares timely and accurate notes of employee issues and meetings.

o Evaluates resumes or employment applications of potential candidates for open positions.

o Assists with orientation and training of newly hired staff. Develops on-boarding and training schedule for new team members.

o Facilitates the professional development of staff. Accepts responsibility for maintaining and updating knowledge of all new and existing policies and procedures and participates in education of staff.

o Holds and leads routine staff meetings, clinical or non-clinical, to communicate key information.

o Determines training and development programs for support staff and arranges opportunities for education.

o Implements and assists in the training and orientation program for all new employees.

o Initial recipient of ?call outs?. Assist with the daily schedule to cover ?call-out?s?, and then reports the changes to the Center Manager/Center Director. Keeps the flow of the work running smoothly, and reports any issues that may impede the work environment to the Center Manager/Center Director.

  • Coordinates/evaluates daily operations including clerical, nursing, information systems, and facilities management. Provides first-line support and resolution of facility issues.
  • Evaluates key processes, clinical or non-clinical, for continued improvement and efficiency.
  • Recommends facility improvements and routinely evaluates clinical and non-clinical space.
  • Maintains all necessary center information brochures, accreditation documents, and other information.
  • Assists with ensuring compliance with local, state, and federal rules and regulations, and accrediting bodies. Conducts and participates in real or mock surveys and or emergency drills.
  • Initiates incident reports and ensures proper completion and communication as required.
  • Functions as a Subject Matter Expert for EHR, vaccine coordinators, and Florida Shots Program or Clinical programs.
  • Ensures equipment is in proper working order; including required yearly or bi-annual maintenance stickers.
  • Performs duties in assigned position and may fill in if there is an absence of/or shortage of staff.
  • If assigned, responsible for ensuring the QA log, refrigerator/freezer temperature log, eye wash station log, reportable disease log, and Pap smear logs are being completed according to the recommended standards of practice.
  • If assigned, monitor that instruments are properly cleaned, sterilized and stored for procedures.
  • Acts as a liaison between departments regarding center business and/or clinical functions as applicable.
  • Maintain supply inventories and may order supplies with leadership approval.
  • Completes additional projects and/or duties as assigned within established guidelines.
  • Perform other duties as assigned.

Qualifications:

Education:

  • High school diploma or equivalent is required.
  • Bachelor?s degree preferred.
  • Licensed Practical Nurse or Registered Nurse is required.

Certification/Licensure:

  • Must possess and maintain valid Florida license.
  • Current BLS certification required or must be obtained within the first 30 days of employment.

Experience:

  • At least 2 years previous experience in a healthcare setting (i.e. community health center, large physician or group practice) required.
  • Previous supervisory experience preferred.
  • Knowledge of EHR software or equivalent is preferred.
  • Typing and computer skills required.

Special skills:

  • Able to create an atmosphere conducive to customer friendly relations by providing memorable, courteous, friendly and prompt customer service.
  • Bilingual (English/Spanish) communications skills helpful and may be required in some instances.
  • Ability to show courtesy and respect to all customers.
  • Ability to deal with patients effectively in frequently upsetting situations.
  • Ability to be a team player.
  • All written communication and work products must be accurate, professional and reflect the quality standards of CHC.
  • All interpersonal communications and interactions are professional, supportive and respectful, and align with the CHC Code of Conduct standards.
  • Able to function professionally and calmly in emergency situations.
