LPN (Extended Care)-Full-Time, Part-Time, and PRN

Job Summary: The Field LPN will provide services in accordance with home health agency policies, prepares clinical and progress notes, assist the physician and/or registered nurse in performing specialized procedures, prepares equipment and materials for treatments, observing aseptic technique as required, and assists the patient in learning appropriate selfcare techniques.
Essential Duties and Responsibilities:
Administers direct patient services under the supervision of a registered nurse and in accordance with the laws governing the practice of practical nursing
Assists the Registered Nurse or physician in performing specialized duties
Observes and reports the patients response to treatment and any changes in the patients condition to the physician and the supervising RN
Writes all telephone or verbal orders received immediately and turns these in promptly for countersignature by a designated RN.
Administers prescribed medications and treatments as permitted by the laws governing the practice of practical nursing and as ordered by the patients physician
Performs wound care as ordered in accordance with the plan of treatment
Assists with feeding, ambulation and personal care if assigned to do so
Teaches the family or patient procedures under the direction of a registered nurse
Assists the patient in learning appropriate self-care techniques
Performs venipunctures as ordered once competency has been evaluated and documented
Completes a skilled nursing note as services are performed and submits these for incorporation into the clinical record within 24-48 hours unless
Follows the established policies and procedures of the home health agency
Participates in inservice education as required
All other duties assigned

Knowledge of general nursing theory and practice,
Skill in recognizing and identifying symptoms and reactions
Valid Louisiana LPN license
One year of related experience required, two years of related experience preferred
Local travel is required daily for this position
Overnight travel may occasionally be required
Works under the direct supervision of the Case Manager and Assistant Director of Nursing and under the general supervision of the Director of Nursing
Employee frequently lifts or move objects or patients weighing up to 75 pounds
Must be able to walk, sit, stand, lift, bend and reach with hands and arms
