Licensed Practical Nurse- Community Living Center
These Licensed Practical Nurse positions are located at the VA Medical Center in Bath NY within Community Living Center under the Geriatrics and Extended Care Line. Major duties and responsibilities include, but are not limited to:
- Knowledge and skills necessary for concentrated effort and self-direction in carrying out the most complex practical nursing care assignments, in a variety of settings.
- Provide direct bedside care as assigned by Charge RN or Supervisor.
- Serve as a member of the health care team with these complex patients and provides direct care to them on a regular and recurring basis.
- Perform nonstandard and specialized practical nursing activities requiring broad work experience and demonstrated skill sufficient to resolve a range of nursing problems.
- Serve as a member of the interdisciplinary care team responsible for the coverage of patient transports, as well as Community Living Center and MVAC unit coverage as needed.
- Other duties as required.
WORK SCHEDULE: Full time; work schedule to be discussed during interview process.

- Licensure - Must possess a current LPN licensure (copy of licensure is required)
- English Language Proficiency - Must be able to speak and write basic English language (per 38 U.S.C. 7402(d).
- Education- graduate of a school of practical or vocational nursing approved by the appropriate state agency, and accredited by the National League of Nursing Accrediting Combustion (NLNAC) or have graduated from a foreign school of nursing (copy of transcripts are required)
Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religions; spiritual; community; student; social). Volunteer work helps build critical competencies, knowledge, and skills and can provide valuable training and experience that translates directly to paid employment. You will receive credit for all qualifying experience, including volunteer experience.
Past work experience in a Long-term Care facility, Hospice care, Rehabilitation unit, or Dementia Care arena.
GS-5 - Completion of at least 1 year of qualifying experience at the GS-4 level or equivalent. This includes demonstrated knowledge and ability to provide a full range of practical nursing care to patients with a variety of physical and/or behavioral problems; demonstrated ability to serve as a responsible member of the nursing team and interact, in an appropriate manner with patients, family members, professional and other supportive personnel involved in the delivery of patient care, incorporating acceptable, established customer service standards into practice; knowledge and skill sufficient to prepare, administer, and appropriately document actions taken specific to commonly prescribed oral, topical, subcutaneous, intramuscular, and/or intravenous medications as permitted by approved local facility policies and procedures; knowledge and ability to recognize the need for and to institute emergency measures when indicated, promptly seek the assistance of the RN or MD/DO, and assist in resuscitation procedures in cardiac and/or pulmonary arrest; recognizes and appropriately responds to breakage/malfunction or loss of equipment, safety hazards, and supply deficiencies, promptly reporting to appropriate personnel for corrective action OR completion of a two year practical or vocational Nursing program OR a combination of education and experience.
