What You Will Do:

General Summary

Under general supervision, provides and coordinates the care provided to surgical transplant patients. Serves as a fully effective and participative member of the multidisciplinary patient care services team. Demonstrates and models the full range of nursing professional practice in the delivery of patient care and in participation of the development of strategies to improve patient outcomes, satisfaction with service and cost effectiveness. Is accountable to patients and families for the processes and outcomes of care during an episode of care. Is accountable to peers for the advancement of group practice within the Department of Transplantation.

Principal Responsibilities and Tasks

The following statements are intended to describe the general nature and level of work being performed by people assigned to this classification. These are not to be construed as an exhaustive list of all job duties performed by personnel so classified.

Clinical Practice/Patient Care

  1. Provides direct patient care to and coordinates multidisciplinary care for surgical patients. Models high level of clinical competency and expertise related to patient populations in the surgical clinic/procedural setting.
  2. Serves as a resource in planning the care for complex surgical patients. Identifies need for and initiates patient care as directed by the physician.
  3. Working directly with the multidisciplinary team, manages care of surgical patients. Triages all outpatient phone calls, guiding patient care through coordination of multidisciplinary efforts.
  4. Models relationship skills with patients and families to prevent poor outcomes and promote patient satisfaction.
  5. Models effective documentation practices. Develops strategies to ensure complete documentation as it relates to excellent patient care, and also regulatory requirements.
  6. Identifies impact of new technology on patient care outcomes and on workflow processes.
  7. Coordinates scheduling, preparation and outpatient follow-up for surgical procedures as directed by the physician. Completes diagnostic work up/pre operative testing to ensure patients are ready for surgery as indicated by the physician.
  8. Schedules patients for surgery as appropriate, and educates patients as necessary regarding surgical procedure.
  9. Provides outpatient surgical follow up, interacting with all disciplines as necessary
    • Coordinates scheduling of all post hospitalization surgical procedures, diagnostic tests, clinic visits and meetings in a medically and logistically appropriate order.
    • Coordinates all outpatient lab work, x-rays, and medicine changes as directed by the physician/multidisciplinary team.
Provides direct support to patients/family members as identified.
  1. Coordinates admission to hospital when necessary as directed by the physician.
  2. Notifies multidisiplinar team members of potential surgical admission, including admitting personnel and accepting unit resident/NP. Communicates any physician recommendations/orders to appropriate staff.
Coordinates data collection for surgical patients.
  1. Develops and maintains communication and record keeping systems to facilitate patient referral, acceptance, and management.
  2. Obtains and organizes patient information in database.
Provides appropriate nursing units with transplant educational materials and continual updates as needed or required.

Participates in patient-related consultations with physician staff as requested, including attendance at Team meetings.
  1. Participates as team member in Team Meetings.
  2. Discusses surgical candidacy with Team members/Physician
Establishes and maintains frequent contact with referring physicians and institutions to ensure the timely transmission of pertinent medical records, pathology slides, and diagnostic test results. Communicates same to medical staff.

  • Communicates with referring physicians concerning patient eligibility, protocols, surgical candidacy and related issues.
  • Encourages frequent communication with referring physician immediately post-surgery.

What You Need to Be Successful:

Education & Experience:

  1. Bachelors degree required; BSN preferred.
  2. Licensure as a Registered Nurse in the state of Maryland, or eligible to practice due to Compact state agreements outlined through the MD Board of Nursing, is required.
  3. Three years of recent applicable nursing experience is required. Critical Care, surgical specialty, renal or Transplant experience is preferred.
  4. Must obtain NATCO certification within 2 years of hire, or within a timeframe determined by management.

Knowledge, Skills and Abilities

  1. Demonstrated effectiveness as a multidisciplinary team member.
  2. Demonstrated ability to plan, organize and follow through on assignments and/or projects. Ability to analyze and conceptually develop ideas and proposals.
  3. Ability to learn and use computer systems in order to manage patient information.
  4. Ability to demonstrate knowledge and skills necessary to provide care appropriate to the patient population(s) served. Ability to demonstrate knowledge of the principles of growth and development over the life span and ability to assess data reflective of the patient's requirements relative to his or her population-specific and age specific needs.
  5. Demonstrated/documented highly effective interpersonal and verbal and written communication skills.
