As part of our continued growth we are searching for a Full-time Nurse Practitioner to join our Phoenix team.  

Position Highlights:

  • Medically manage and educate the Senior Diabetic Population in Optum Community Center Wellness Clinics
  • Multi-faceted approach will improve the lives of our diabetic members
  • Provide comprehensive medical and laboratory evaluation
  • Complete full patient assessments on our Medicare Advantage Senior Patient Population
  • Formulate a list of current and past medical conditions using clinical knowledge and judgement, and the findings of your assessment
  • Identify diagnosis to be used in care management and active medical management in the furtherance of treatment
  • Order and evaluate medically appropriate screenings
  • Screen for drug/drug, drug/food, drug/disease interactions and adverse reactions
  • Identification of barriers to optimal care
  • Education and/or intervention including but not limited to:
    • Medications, regular monitoring of both self-tested and laboratory tested blood glucose levels
    • Educate patients on use of home blood glucose monitoring equipment and home blood pressure monitoring
  • Monitor patient compliance with prescribed therapies and scheduled clinic and laboratory appointments
  • Referrals to Podiatry, Nutrition, Ophthalmology, and Endocrinology as needed
  • Collaborate with Primary Care providers via medical record and verbal communication as needed
  • Monthly IDT to review enrollees or those with specific challenges
  • You will see on average 7-8  senior diabetic patients a day
This position involves care at the Optum Community Center Wellness Clinics, telehealth and potentially in the home based, so some travel around Maricopa County will be required, and you will have to be comfortable working autonomously.

Required Qualifications:

  • Arizona Nurse Practitioner license and national certification (Family Medicine or Geriatric, or Adult Medicine Certified), or the ability to obtain an Arizona Nurse Practitioner License
  • Active DEA license with prescriptive authority
  • >5+ years of clinical experience in their highest level of education
  • Proficient in assessment and development of treatment plans; specifically related to endocrinology and geriatric medicine
  • Arizona Driver’s License and Auto Insurance in accordance with the state of Arizona (or will be licensed and insured in Arizona by employed date)
  • Ability to lift a 30-pound bag in and out of a car to navigate stairs and a variety of clinical spaces
  • Ability to work autonomously
  • Full COVID-19 vaccination is an essential job function of this role. Candidates located in states that mandate COVID-19 booster doses must also comply with those state requirements. Preferred Qualifications:

  • Experience in Endocrinology, Gerontology, or Internal Medicine, and Cardiology
  • Home care or home visit experience
  • Knowledge and or experience working with the Medicare Advantage Population performing patient assessments
