The Transition Placement Coordinator is responsible for discharge planning and transitional Care Coordination for high-risk members in Inpatient settings who have been involuntarily detained with complex needs which are presenting barriers to discharge.   This position integrates medical and behavioral health in a collaborative process which assesses needs, plans, implements, coordinates, monitors, and evaluates options and services to support discharge from inpatient levels of care to less restrictive settings, with the necessary array of services to support community tenure.

Primary Responsibilities:

  • Care Coordination and discharge planning for members in behavioral health care settings who are likely to experience significant challenges to discharge 
  • Performs comprehensive assessments to facilitate discharge planning, identifies and assists members with high-risk symptoms/diagnoses and SDOH needs or members with multiple co-morbidities who will benefit from intervention and engaging in care management services. Information is collected from the patient, caregiver(s), health care providers and other relevant parties as needed
  • Facilitates discharge planning efforts with timely, streamlined communication across patient’s care teams to include case conferencing as needed to identify and obtain necessary services and supports for safe discharge or transition to facilitate improved outcomes
  • Integrates a collaborative approach by initiating and/or attending interdisciplinary team meetings with nurses, physicians and patient care coordinators, Allied Service system providers (HCS, DDA, etc.) and outpatient providers regarding patient care as needed. Collaborates with providers to determine acuity of behavioral health concerns and refer members to appropriate community resources or stepdown levels of care
  • Coordinate with the Utilization Management Care Advocate, monitor utilization and provide feedback on planned interventions and outcomes of the plan of care
  • Coordinates with facility discharge planner/social worker to ensure individualized comprehensive discharge plan including assessing for social determinants of health impacting member’s likelihood of success in the community 
  • Utilizes advanced clinical assessment and treatment planning skills to effectively leverage clinically appropriate outpatient services and supports to meet the member’s health, behavioral health and psychosocial needs., providing coaching, patient education, communication, and all available resources to promote quality and cost-effective outcomes
  • Documents patient/family status, diagnosis, medications, discharge and transition plans, interventions, evaluation results, observations, and progress in electronic medical record/proprietary database in a timely and accurate manner
  • Advocates for members and families as needed to ensure the patient’s needs and choices are fully represented and supported by the health care team 
Hours for this role will be Monday - Friday (8:00 am - 5:00pm).

Required Qualifications:

  • Licensed Master’s degree in Psychology, Social Work, Counseling or Marriage or Family Counseling, or Licensed Ph.D., or an RN with 2+ years of experience in behavioral health
  • Licenses must be active and unrestricted in State of Washington
  • 2+ years Post Education (Master’s degree experience in a related mental health environment)
  • Designated workspace and access to install in-home secure high-speed internet via cable/DSL
  • Proficient Microsoft skills (Word, Excel, Outlook)
  • Full COVID-19 vaccination is an essential requirement of this role. Candidates located in states that mandate COVID-19 booster doses must also comply with those state requirements. Preferred Qualifcations:

  • Experience in Community Mental Health services
  • Preferred Master’s degree for RNs
  • Managed Care experience 
  • Dual diagnosis experience with mental health and substance abuse 
  • Experience working in an environment that required coordination of benefits and utilization of multiple groups and resources for patients
  • Experience working with the Medicaid population
  • Experience working with low-income populations
