Hospice of the Valley provides comprehensive and compassionate end-of-life care for the terminally ill as well as support services to the family. Hospice of the Valley works to increase community awareness of death and grief as a natural part of life.

The Nurse Liaison holds a pivotal role in interfacing with physicians, clinicians, social workers, discharge planners, families and patients. An essential requirement for this position is the public relations component due to the situations within the hospitals that require a professional presence. The Liaisons primary responsibility will be hospice care and information at HMHP facilities (SEHC, SEBHC, SJHC), however may also provide services at other area hospitals and subacute care facilities.
Is a graduate of an accredited school of professional (registered) nursing. BSN preferred.
Is currently licensed as an RN through the Ohio Board of Nursing.
Demonstrates exceptional interpersonal and communication skills. Must be able to deal with diverse groups  community, volunteers, and a variety of health care providers, physicians, discharge planners, social workers, case managers and Agency staff. Travel required.
Must be able to handle multiple projects at one time. Demonstrated ability to develop, implement and meet goals and objectives. Must be a self-starter.
Possesses and maintains current CPR.
Has a valid drivers license and is insured in accordance with state and/or organizational requirements.
Act as a liaison with referrals within the hospitals to facilitate a smooth transition from acute care to hospice care either as inpatient admission, transfer to Hospice House, or to discharge home/ ECF/ALF. Information about referrals will be coordinated with Hospice Intake Department. Excellent organizational skills are required to facilitate and streamline referral calls to service the hospital in timely discharges per their timeline; and also to coordinate this with family needs and hospice staff availability. Accurate documentation as to date, time and telephone contact of each action and visit will be kept. Updates will be provided daily and on an ongoing basis.
Speak with physicians when futile care is ordered to educate them on palliative treatment options versus aggressive acute modalities of care. Primarily, the best interest of the patient with regard to hospice philosophy is the goal. Pursue orders to discontinue aggressive treatments when appropriate.
Liaison may be scheduled various shifts including weekends and holidays. Must be available during regular hospice hours of operation. Extra time will be required at times to accommodate all referrals. The goal is to complete referrals the day the order is written during scheduled work time.
The Nurse Liaison will work closely with the Agency Medical Director in meeting the needs of hospitalized patients.
Must provide public relations and marketing component without difficulty as the opportunity arises. Excellent documentation skills required within the hospital and hospice electronic medical record.
The Nurse Liaison will be responsible for the case management of inpatient admissions. The nurse will also serve as a liaison for home hospice patients required a hospitalization for symptom control or respite stay.

All personnel must attend mandatory educational programs.
