Description:Assists in coordination of Home Health Care/Hospice Services to patients being transferred from area hospitals to St. Mary’s Home Health Care/Hospice Services. Assists in care coordination of current patients in Home Health Care/Hospice from area hospitals to home. Serves as a resource person to physicians, patients, families, and staff. Provides information about Home Care, Hospice and community services to St. Mary’s Home Health Care patients/families.

RN with current Georgia license is required. Current drivers license for state of residence and insurance as required by law. Must be a graduate of an accredited school of nursing. B.S.N. desirable. A minimum of 3 years experience in Nursing is required. At least one year of home care or hospice experience preferred.

Position requires a person who is responsible, ethical and professional. Must have effective interpersonal skills and be able to communicate with a diverse population. Must be able to cope with stressful work and be flexible. Must be able to work independently and efficiently. Must be able to recognize and deal with priorities. Must have excellent verbal and written communication skills. Must be comfortable with discussing end-of-life issues with patients, families and staff.
