Objective
This position supports the Care Coordinators to ensure that interoffice operations and patient management activities are coordinated timely and efficiently to support the clinical team. The Supervisor, Care Coordinator is responsible for the team’s performance in customer service, schedule management, patient visit reminder calls, and management of consents and medical records, disenrollment, and hospice related activities. You will also support Landmark Physicians, Advance Practice Providers, and RN Nurse Care Managers with important operational and clinical tasks. You will draw on your critical thinking, clinical experience, and your positive attitude to coordinate with and support Landmark patients and Landmark team members.
Primary Responsibilities:
Demonstrate a high level of customer service behavior at all times
Maintain HIPAA compliance standards
Expedite emergency calls as needed
Have solid communication skills
Ensure team completes daily tasks
As a critical part of the team the Supervisor, Care Coordinator will:
Have regularly scheduled staff meetings
Support all clinician and Interdisciplinary care team members
Coordinate care per established Care Coordinator procedures, protocols and timelines
Ensure primary care provider preferred method of communication is updated/accurate in the EMR
Gather and relay necessary clinical and administrative information to appropriate members of the clinical team
Complete Patient Satisfaction Calls, as required
Participate in quality monitoring process
Participate in OOM meetings as the topics pertain to Care Coordinator processes or Ubiquity updates
Oversee daily operations (i.e., Daily on-call schedule, medical record uploads, management of consents, mailings, UV schedule, IV Packet readiness)
Supervision of Care Coordinators (i.e., staff schedule, PTO, ADP, Cornerstone training completion)
Timely completion of Quarterly Performance Updates (QPU’s) and annual evaluations for Care Coordinators
Review of Annual Incentive Plan with each staff member to ensure understanding of company goals
Participate in Care Coordinator hiring process, training, performance evaluation, and discipline
Provide regular coaching and constructive feedback to Care Coordinator to enhance overall effectiveness
Assist in development, documentation, and implementation of departmental policies and procedures
Skilled as a super-user of technology (windows programs and Electronic Medical Records (EMR’s)
Skilled as an educational trainer on the Care Coordinator role and function for department and market
Ensure Care Coordinator documentation meets Landmark standards and appropriate abbreviations are used
Review and utilize Reports Dashboard data for management of the department
Maintains relationship with market leadership, especially Supervisors of Clinical Operations to ensure coordination of care, understanding of policy/practice change that impact department or market
Other special projects or initiatives as assigned

General Manager/Executive Director, Health Service Director, Supervisor of Clinical Operations or other local market leadership

Required Qualifications:
High School Diploma or GED
Experience with escalated customer service issue resolution
Demonstrated critical thinking skills and flexibility
Knowledge of MS Office products
Proficiency in Excel

Preferred Qualifications:
Bachelor’s Degree
Experience as a Medical Assistant, Certified Nursing Assistant, Pharmacy Tech, or other clinical or administrative role in a health care setting
Medicare experience
Experience with following triage protocol
Typing speed of 40+ WPM
Physical Demands:
Regular requirement to sit; use hands to touch, handle or feel
Regular requirement to talk and listen
Occasional requirement to stand; walk and reach with hands and arms
Vision abilities include close vision, peripheral vision, depth perception and ability to adjust focus
