Southwest Medical, part of OptumCare, is a multi-specialty group of physicians, Nurse Practitioners, and Physician Assistants consisting of over 300 providers, 21 medical health centers including six urgent care clinics, four convenient care centers, two lifestyle centers catering to older adults, an outpatient surgery center, home health, hospice and palliative care services,  plus E-visits and online options via Now Clinic telemedicine technology.  Southwest Medical offers patients compassion, innovation, and quality care throughout southern Nevada. Primary Responsibilities:

  • Provide physical therapy services to patient according to physician’s plan of care. This may include, but will not be limited to assessing and evaluating therapeutic / rehabilitative / functional status, participation in the development of the total plan of care and evaluates home environment
  • Initiates physical therapy program and instructs other personnel and / or family caregiver members in certain phases of physical therapy with which they may work with a patient, as well as instructing them as to the goals of the physical therapy program for the patient by participating in case conferences
  • Prepares and submits clinical and progress summaries in a timely manner
  • Participates in discharge planning for patient
  • Supervises physical therapy assistants according to organization policy and state regulations, treat and re-evaluation patient no less than every 7th day of treatment or 21 days whichever occurs first
  • Maintains contact / communication with other personnel involved in the patient’s care to promote coordinated, efficient care
  • Maintain clinical competency in physical therapy practice and theory
  • Will participate in Agency on-call coverage as outlined in Agency policy

Required Qualifications:

  • Graduate Degree from an Accredited Physical Therapy Program
  • Possess an active and unrestricted Nevada Physical Therapist license
  • 2+ years of experience as a Physical Therapist
  • CPR certification or obtain within 30 days of hire
  • Access to reliable transportation that will enable you to travel to client and/or patient sites within a designated area
  • Full COVID-19 vaccination is an essential requirement of this role. Candidates located in states that mandate COVID-19 booster doses must also comply with those state requirements. UnitedHealth Group will adhere to all federal, state and local regulations as well as all client requirements and will obtain necessary proof of vaccination, and boosters when applicable, prior to employment to ensure compliance
Preferred Qualifications:

  • 1+ years of Home Health experience
  • Experience with Electronic Medical Records Systems
  • Experience with OASIS documentation
  • Experience in a home health setting
  • Weekend availability

Nevada Residents Only The salary range for Nevada residents is $72,800 to $129,900.  Pay is based on several factors including but not limited to education, work experience, certifications, etc. In addition to your salary, UnitedHealth Group offers benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements).
