We are seeking a PPS Intake Review Coordinator for our Skyline Rehab Program to ensure that we continue to provide all patients with high quality, efficient care. The Rehab Center at TriStar Skyline is Middle Tennessee's only CARF accredited acute inpatient physical rehabilitation program. Our program provides specialized and highly skilled physical rehabilitation services to patients with complex needs.

Rehabilitation begins early in the patient's recovery. A dedicated rehabilitation team consists of physical therapists, occupational therapists, speech therapists and rehab nurses. Each patient's rehabilitation plan is individualized to help patients and their families achieve the best possible outcome and quality of life.

The 41-bed unit is one of Middle Tennessee's largest premier inpatient facilities featuring large, private rooms. Our team of board-certified physical medicine and rehabilitation physicians works with patients and their families every step of the way to get patients back to the quality of life they deserve.

Job Summary:

    Thoroughly researches clinical documentation for evidence of co-morbid conditions.  Verifies accuracy of coding under direction of attending Rehab physician. Responsible for staff education to improve CARE tool score accuracy. Reviews clinical documentation and makes recommendations to clinical supervisors in areas needing improvement to support and validate rehab criteria compliance.  Oversees/completes and ensures the accuracy and the completion of the Inpatient Rehabilitation Facility-Patient Assessment Instrument (IRF-PAI) prior to electronic transmission. Serves as an information source and educator to IRF-PAI users. Collaborates with physicians and interdisciplinary team members to make the most accurate assessment of the needs and strengths of patients. Maintain a current knowledge of the PAI software and provides expertise on this software within the inpatient rehab setting. Provides ongoing education to the staff of the hospital to ensure accurate and timely completion of the PAI. Maintain updated and ongoing knowledge of the regulations concerning the PAI and its completion within the inpatient rehab setting. Verifies appropriate CMG designation through chart review, staff interview and collaboration with Rehab Medical Director Coordinates the completion of the admission assessment of the PAI by the fourth (4th) day of the patient’s stays and the fifth (5th) day following discharge for each inpatient rehabilitation patient.  May require discussions/interactions with various staff members concerning differences in reported functional status to complete the PAI.  Submits completed IRF PAI to HIM Department for coding transmittal. Periodically audits PAI, clinical documentation and billing information to ensure accuracy and consistency of documentation. Works closely and collaboratively with all members of the clinical team to ensure compliance at all levels. Demonstrates effective critical thinking skills.

Our Rehabilitation Program is designed to help people overcome the effects of a stroke or other debilitating injury or illness. The program combines physical therapy, occupational therapy, speech-language pathology, rehab nursing and neuro-psychology services under the direction of a physiatrist (physical medicine and rehabilitation physician). Education, disease management, diet, home set-up, social and emotional needs and home exercise programs are provided to patients and their families to help them manage their care and perform acts of daily living at home. Medicare and most private insurance plans cover some or all of prescribed rehabilitation services.

Qualifications

    Two or more years of experience as a licensed clinician in an acute hospital or rehabilitation setting preferred Ability to operate basic computer and data management software CARE tool credentialed within 1 year of being hired.
