The RN Case Manager is responsible for planning, coordinating and facilitating necessary services for patients. It is vital that Registered Nurse Case Managers focus on high quality medical services and efficient processes for the patient and organization.ResponsibilitiesA RN Case Manager's duties include:
Meeting with the patient and their family to determine their needs.
Creating strategies to satisfy the patient’s needs.
Providing support to the patient and their family members.
Help in locating the lowest cost method of care.
Ensuring the plan is meeting the patient’s needs.
RequirementsOne year of experience as a RN Case Manager within the past two years.
Current Nursing License in good standing.
Current BLS from American Heart Association.
Negative Drug Screen.
