*LOCATION: Norfolk, VA *
*Primary Function/General Purpose of Position*
To coordinate care, resources, and activities for the oncology patient population as part of the Nurse Navigation Program.Recognizing the complexity of the health care system often leaves patients and significant others feeling confused, lost or alienated, the nurse navigator helps guide the patients through the various services, specialties, and resources involved in cancer care. This integral position is part of a comprehensive cancer program designed to decrease fragmentation of care and coordinate service. In addition to supporting the clinical and educational needs of the patient, the navigator will enhance patient access and ease referral and management for physicians. The nurse navigator creates seamless flow through the health system such that the patient can focus on fighting his/her disease.
*Employment Qualifications*

* Graduate of an accredited school of professional nursing.
* Current registered nurse licensure to practice nursing in the Commonwealth of Virginia. Bachelors degree in nursing preferred.
* Minimum of three years clinical experience preferable in Oncology. Oncology Certification preferred; if does not presently, must have with 1 year of hire.
* Must be self-motivated and have the ability to work independently. Knowledge of windows-based computer systems and software.
* Strong relationship with oncologists and specialty providers; highly knowledgeable of medical terminology, cancer as a disease process and the various treatment options; highly knowledgeable of the environment and system through which the patient must move in order to obtain care; highly knowledgeable of community resources and programs or of how to obtain such information; culturally attuned to the people of the community being served; excellent communication skills, flexible, sensitive and compassionate.
*Essential Job Functions*

* Delivers patient care in accordance with the ANA Scope and Standards of Care and Practice, Patient Bill of Rights and the ANA Code of Ethics for Nurses.
* Delivers patient care in accordance with the Oncology Nursing Society, Standards of Patients Care

* Demonstrates knowledge and skills specific to the position.
* Provides support and counseling to patients and their families in the nurse navigator role.
* Acts as an advocate for cancer patients.
* Is an active visible healthcare team member in the care of the inpatients and outpatients for the patient population nurse navigator is working with.
* Provides education to patients and families as it relates to the disease process.
* Develops and updates patient educational materials on a routine basis.
* Understands and works to improve patient satisfaction with in the cancer program.
* Stays abreast on current literature and innovations in the field of oncology.
* Works effectively with physicians/disease specific centers/cancer programs to coordinate the support and efficient course of care for the cancer patient.

* The Program Coordinator is responsible for the development, implementation, coordination, and evaluation of the Oncology disease program they are responsible by adhering to the guidelines of the American College of Surgeons or other accrediting body for any program that is certified( i.e. certified breast center-NAPBC.)
* The coordinator demonstrates ability to effectively use applicable computer systems required by the program to formulate and analyze data base reports.
* The coordinator may be required to work at alternate sites.

* Participates in the development and implementation of a comprehensive cancer center/program at the hospital/facility.
* Supports collaboration with other departments to share pertinent information.
* Demonstrates problem solving, conflict resolution and negotiating skills.
* Demonstrates leadership and teamwork, and facilitates positive relations with healthcare team.
* Demonstrates excellent customer service skills. Responds to customers needs and requests promptly and professionally with compassion and caring.
* Supports Cancer Program initiatives including the Commission of Cancer Accreditation, NAPBC accreditation, other accreditations and goals set forth by the Cancer Program Leadership.
* Meets/exceeds nurse navigator and cancer program goals.

* Coordinates interaction and communication between patient and multi-disciplinary team.
* Assists physician in addressing patient questions and concerns.
* Attends multidisciplinary cancer conferences/tumor boards,
* Coordinates conferences and tumor boards and ensures program compliance with standards.
* Ensures timeliness in scheduling and retrieving patient results.
* Assists patients in keeping follow up appointments after treatment is completed.
* Promotes informed decision making.
* Acts as an advocate for the oncology program to outside agencies and networks.

* Supports data collection and metric development for cancer service line
* Tracks metrics and reports monthly statistics
* Manages patients through Connect Care and EquiCare ensuring that patients are monitored throughout their active treatment.

* Facilitates community outreach and education events in support of the cancer service line. Connects patients it related community, support, financial, and collaborative resources.
* Help patient identify and access financial services to help pay for their health care needs and utilizes appropriate services in a timely manner.
* Connects patient to community resources and support services.
* Make referrals to consultants in collaborative care: nutrition, social work, rehabilitation, palliative care, hospice, etc., as appropriate

* Complaint/Competent wit National Patient Safety Goals applicable to this role.
* Employee is either compliant with NPSGs (for clinical employees) or competent in their knowledge of NPSGs (for nonclinical employees).

* Patient Population: Provides individualized care to address the needs of the patient including: Physical, psychological, socio-cultural, spiritual and cognitive needs.
* Demonstrates effective use of the nursing process to deliver individualized care to patients:
* Performs patient assessments through systematic collection and review of patient-specific data; and communicates assessments appropriately.

* Assessments consistently reflect physical, psychological, socio-cultural, spiritual, and cognitive needs as well as functional abilities.
* Plans care based on the assessment, evaluates patient progress, and revises care to attain expected outcomes.
* Assess patient/family/significant other learning needs, plans and provides education and evaluates the effectiveness of teaching in achieving desired outcomes.
* Provide education and care advise, in person and via telephone including but not limited to:
* Pre- and post-operative care, multiple surgical procedures, diagnostic testing, medications, radiation treatments, chemotherapy, immunotherapy/biotherapy and anti-hormonal therapy

* Identifies and responds appropriately to the ethical issues in patient care; provides nursing care in a non-judgmental manner that respects patient diversity and acknowledges patient rights.
* Integrates principals of age-specific care into aspects of practices.

* Demonstrates excellent interpersonal skills with focus on customer service, patient satisfaction and effective relationships with physicians and other ream members.
* Knowledgeable in performance improvement and continuous improvement concepts and utilizes these skills in problem identification and improvement in work processes and outcomes.

* Demonstrates financial stewardship and manages areas of responsibility within budget. Serves as a role model for professional and clinical practice.
