Job Summary:

In this role you will have the opportuntiy to facilitate the interdisciplinary team to plan, implement, and evaluate an individualized plan of care based on the needs and available resources for our behavioral health patients at TriStar Summit Medical Center.  You will be responsible for the all activiteis of the behavioral helath program to ensure the highest quality of care in a safe and therapeutic environment. This role will support our patients in the ED or admitted for medical reasons who also are faced with behavioral health diagnosis.  

  • You will collaborate with medical and hospital staff to support care for patients with behavioral health diagnosis
  • Your responsibilities will include rounding with Physicians on patients with behavioral health diagnosis, reviewing clinical data, and making recommendations for management of patients needs
  • As the Coordinator you will be involved in chart auditing, identifying trends, leading process improvement initiatives for our patients throughout the hospital experiencing behavioral health symptoms.  
  • You will identify and integrate patient learning needs, abilities, and readiness into the plan of care
  • You will work in collaboration with the nursing team to develop and implement an individualized paln of care
  • You will ensure that protocols become the standard of care for behavioral health patients around the hospital and in the Emergency Depts. 
  • You will be familiar with the mission, vision, and value statements of this facility.

Qualifications

  • Registered Nurse, State of Tennessee Licensure or Compact Licnesure
  • Bachelors Degree Preferred
  • Behavioral Health Experience STRONGLY Preferred
  • Knowledge of performance improvement tools and methodology
