In order to help our support services team, we are looking for a Nursing Supervisor to lead our Case Management &Support Services department. Working in conjunction with our Social Work and Navigation Team, you will lead our Patient Navigators in providing resources, information, and support for patient’s and their families as they navigate the Cancer Care Continuum. Experience in Oncology is not required, but strongly preferred along with knowledge/experience in Case Management, Team Leadership, Behavioral Health, Palliative Care, and/or specialty medicine.

JOB SCOPE:        

Responsible for supervising dieticians, social workers, patient navigators, and nurse navigators who provide individual assistance to patients, families, and caregivers to overcome health care system barriers and provide timely access to quality healthcare within a community oncology setting. Utilizing the nursing process, provides patient education and access to relevant internal and external resources. Primary objectives are to facilitate informed decision making and timely access to high quality health and psycho-social care through all phases of the cancer continuum. Routine collaboration with internal departments and external referral resources are anticipated, including scheduling, financial counseling, lab, pharmacy, social work, nursing, surgical, radiation and diagnostic imaging, dietary, and others. Regular and frequent interface with practice-based leadership teams, physicians, and clinical staff is required to promote and engage the patient navigation process. Supervises nursing and clinical support staff that provides professional nursing care to patients and adheres to national and company standards and guidelines for specialty regulations. Must recognize physical, psychological, and spiritual aspects of care and participates in companywide quality initiatives.


Responsibilities

    Oversees the clinical operations for all nursing clinical support activities.Maintains effective staff coverage for the practice within budgetary constraints.Assists in determining staffing requirements.Screens and interviews applicants and orients and trains staff.Participates in the interviewing and recruiting of personnel.Assists with the review and revision of clinical policies and procedures.Assists with annual performance evaluations for department staff.Demonstrates working knowledge of applicable labor laws and schedules and coordinates employee staffing including vacations, sick leave and overtime.Assists with staff training and in-services as required.Ensures nurses maintain all required competencies on an annual basis.Performs coordination of care during the full continuum of cancer care experienceIdentifies potential and real gaps in care delivery; seeks out opportunities for avoidance and/or closurePerforms assessment of patient risk stratificationPerforms basic psychosocial assessments and provides emotional support; delegates and/or makes referrals for additionalscreening and support as neededDevelops appropriate nursing diagnoses and corresponding action plans as neededSupports and/or provides oral chemotherapy education, adherence and side effect monitoringHolds or participates in survivorship and advance care planning visits; makes referrals as neededParticipates in research, quality, and performance improvement, including reporting on navigation metricsSupports patient advocacy and empowermentParticipates in community outreach and cancer prevention educationKnowledgeable of practice operations guidelines, practice organization and healthcare economic

Qualifications

    Graduate of an accredited school of nursing, BSN or MSN preferred. Minimum five years actively practicing as a registered nurse, three years oncology/high level specialty experience strongly preferred. One year supervisory experience required. Oregon State RN License.Current CPR certification required. Current or pending oncology certification (OCN) strongly preferred.
