Patient Care Manager - RN - Home Health
Heartland Home Health Care a part of HCR ManorCare/ProMedica that provides a range of services, including skilled nursing care, assisted living, post-acute medical and rehabilitation care, hospice care, home health care and rehabilitation therapy, is currently searching for an experienced RN for a Patient Care Manager position. This is an exciting opportunity for an experienced RN to become a part of a growing home health agency.
The Patient Care Manager is responsible for the supervision, orientation, instruction and on-going evaluation of all patient care staff who are engaged in the delivery of care. The Patient Care Manager assures compliance with agency policies and procedures, state and federal laws, regulations and accreditation standards. Included in those responsibilities are providing day to day direction to their teams, administering performance reviews, coaching and providing disciplinary action when needed. The role is also accountable and responsible for driving successful clinical outcomes of their teams and will have a scorecard developed to measure this success. They may have responsibility for the work flow within Home Care Home Base.

Minimum of one year experience in home health care, or hospice nursing preferred, may be specified by state.
Previous management or supervisory experience preferred.
Working knowledge of Home Care or Hospice Conditions of Participation
EducationLevel : Currently licensed as a Registered Nurse (RN) in good standing with the State Board in which he/she practices.
