Project HEAL/Transitions Nurse (KY HEALing Communities Study)

Full-Time (40hrs/wk)

Join our team to be a part of National Institute of Health’s historic HEALing Communities Study, awarded to the University of Kentucky, to reduce opioid overdose deaths by 40% in the next 3-years.

This position is a new line of service through transitions for Opioid Use Disorder with the UK HEALing communities.

QUALIFICATIONS 

  • Is a graduate of an accredited school of professional (registered) nurses, BSN preferred.
  • Is currently licensed as a Registered Nurse through the State Board of Nursing.
  • At least two year’s general nursing experience with addiction, home health, medical surgical, psychiatric, mental health care experience. Transition/Navigation nursing experience is preferred.
  • Understands evidenced-based transitional/navigation services to prevent 30-day avoidable hospital re-admissions, emergency room usage and improve patient outcomes.
  • Complies with accepted professional standards and principles.
  • Must have excellent observation skills, good nursing judgement and communication skills. Ability to provide good written documentation in a timely manner.
  • Demonstrates the ability to work with interdisciplinary team.
  • Is self-directed with the ability to work with little supervision.
  • Must have excellent collaboration skills in working with UK HEAL leadership and team.
  • Skilled in nursing practice, able to cope with family emotional stress and tolerant of individual lifestyles. Knowledge of disease processes, addiction medicine, symptom management, coaching process and community and health resources.
  • Knowledge and basic understanding of the interpersonal dynamics operative within the working relationships of an interdisciplinary healthcare and research team.
  • Current Kentucky driver’s license and unlimited access to a reliable automobile.
  • Ability to travel as needed to all transition sites.
  • Ability to exercise judgement and discretion in confidential matters.
  • Must have excellent written and oral communication skills. Should be comfortable in public speaking with varied audiences.
  • Possess the ability to manage and interpret data.
  • Ability to interact with hospital/partner top leadership in regular meetings.
  • Experienced with providing effective education, feedback and direction with clients and families.
  • Experienced with working with office support staff.
  • Must have strong organizational and planning skills.
  • Possesses and maintains good physical stamina and mental health. Has presented a pre-employment health clearance including a negative TB skin test and/or CXR and other tests as required by organization policy.
  • Possesses and maintains current CPR certification.
  • Must be flexible and can manage multiple tasks and programs simultaneously.
     

SUMMARY OF JOB RESPONSIBILITIES

An employee in this position is responsible for providing clinical nursing and counseling for clients and families in the HEAL program. Responsible for transition/navigation services in the community and home. The position utilizes the clinical navigation protocols for the Study, and is responsible for working with leadership in data collection, management and evaluation.

RESPONSIBILITIES AND DUTIES 

  • Works closely with Project Clinical Team to develop and implement the transition intervention.
  • Assists in developing and coordinating relationships with partners, which includes insurers, hospitals, nursing facilities, physicians, etc.
  • Assists in implementing all policies and protocols for the program.
  • Assumes responsibility for clinical navigation in collaboration with the UK HEAL team.
  • Coordinates referrals with community resources and continually looks for additional resources for navigators.
  • Participates in the quality improvement program.
  • Works closely with the IT department on the electronic medical record implementation and ongoing changes needed for the program.
  • Participates as needed in additional training and education.
  • Attends Study partner conference calls, in-person meetings and presentations.
  • Monitors accuracy of routine required submissions to partners.
  • Openly displays cooperation in adhering to administrative policies and supports the mission of Bluegrass Care Navigators.
  • Assures that work and benefit hours are appropriately documented.
  • Reports to work as scheduled. Does not abuse paid time off, is not tardy, and maintains reasonable flexibility in work schedule.
  • Demonstrates a time-conscious attitude and consistently strives to use time effectively by completing assignments in an efficient, well organized, and self-directed manner.
  • Possesses and maintains current CPR certification.
  • Participates in interviewing and hiring transition coaches.
  • Assists with regulatory compliance issues.
  • Maintains dress code and evidences good grooming.
  • Attends mandatory in-services.
  • Participates in organizational management meetings.
  • Other duties as assigned.
