Care Manager

Responsbilities: The Care Manager is accountable for the management and coordination of the patient's plan of care beginning with the admission, active phase of care, and addressing of discharged needs and concerns to ensure the patient receives appropriate, cost effective, quality care during their in-patient stay.

Requirements:  Graduate of an accredited Registered Nurse diploma or college program,  five or more year's of progressive hospital experience. 2-3 years of case management, utilization reviews or discharge planning experience highly preferred.  Good oral and written communications skills.  Good interpersonal skills.  Licensed and currently registered or otherwise duly authorized to practice as a professional nurse in New York State.  PRI certifications  and the Case Management certification is strongly encouraged.
