Heartland Home Health Care and Hospice, a subsidiary of HCR ManorCare, is currently searching for a full-time Home Health Hospice Aide to provide assistance to our in-home hospice patients in the Washington/Ozaukee/Waukesha/Milwaukee county areas. You will be traveling within a select area of some of these counties (not all) and will be reimbursed for mileage. This is a day shift role working mainly M-F 8am-4:30pm.
Our Home Health and Hospice Aides provide personal care to patients to promote comfort and support. We require a GED or High School diploma, reliable transportation, current Certified Nursing Assistant Certification and at least 1 year of C.N.A experience. - Ability to accurately take and record temperature, pulse, respiration, and blood pressure.
- Ability to accurately observe, report and document changes in the patient.
- Ability to recognize the needs of the patient and family.
- Ability to develop and maintain rapport with patients and families.
- Demonstrated ability to work with an interdisciplinary team
EducationLevel : Minimum of one year experience as a Hospice Aide, or is currently enrolled as a nursing student who has had a minimum of 6 months clinical experience, or has completed a Hospice Aide training program or a certified, state tested nursing assistant (or completion of PCA course in Florida), as allowable under state specific guidelines.
