4609 - Heartland Hospice - Serving Kansas City Metro Area, MissouriA minimum of 6 months nursing experience within the past 3 years unless state regulations differ. Travel is necessary on a daily basis.
Job Specific Details:As a Hospice RN Case Manager, you will manages a patient and family caseload which includes assessing, planning, implementation, evaluating and documenting the care provided in both patient?s home and at living facilities; supervise and document CNA and LPN plans of care and document supervisory visits; participate in on-call coverage; submit paperwork in a timely manner and update and maintain patient medical records; manage patient care with specific knowledge and experience in bedside nursing, symptom management, crisis intervention and family intervention; participate in education and in-service training as requested by the office; maintain proper documentation for billing; manage time efficiently and effectively; currently licensed as a Registered Nurse (RN) and in good standing with the Board in the state of Missouri; minimum of 6 months institutional nursing experience within the past 3 years; lifting 50 pounds maximum with frequent lifting and or carrying of objects weighing up to 25 pounds; walking, standing, talking, hearing, pushing, fingering, reaching, seeing, stooping, smelling, depth perception, color vision, driving necessary on a daily basis
EducationLevel : Currently licensed as a Registered Nurse (RN) in the state and in good standing with the Board in which he/she will practice.
