Seasons Hospice & Palliative Care is seeking a Hospice Nurse-RN IPC Shift Lead to join our team of professionals. We make a difference every day to patients and families coping with end of life illness in creating a center of excellence in hospice care.
Oversee and supervise RN's on the unit to ensure nursing assessment, planning and care to maximize the comfort and health of patients and families in accordance with the interdisciplinary plan of care.
Receive patient assignments from the Director, Clinical Operations and / or Team Director. Schedule and provide daily assignments to RNs on the unit to assume responsibility for a patient/family that includes assessing, planning, implementing and evaluating care. Review CMR daily and address issues. Provide input for 90 Day and Annual Reviews. Complete initial and annual competencies. Conduct B2B visits and provide coaching. Complete chart audits to maintain compliance. Order medications and medication management. Monitor refrigerator temps. Ensure Employee badge compliance. Mitigate any patient and family concerns. Complete Transfers/admissions and patient rounding. Participate in AM REDS meeting and end of day call. Initiate communication with attending physicians, other hospice staff members and other agencies as needed to coordinate optimal care and use of resources for the patient/family. Maintain regular communication with the Team Director, to review staffing issues. Maintain regular communication with the attending physician concerning patient/family care. Ensure unit staff is obtaining data on physical, psychological, social and spiritual factors that may influence patient/family health status and incorporate that data into the plan of care. Ensure unit staff maintains up-to-date patient records so that problems, plans, actions and goals are accurately and clearly stated and changes are reflected as they occur, and all documentation is completed before leaving the IPC at the end of the shift. Accept responsibility for coordinating the physical care of the patient by teaching primary caregivers, volunteers, employed caregivers, and by providing direct care. Ensure staff is instructing the patient and caregiver:
- How to administer medication and recognize side effects. How to perform personal care and oral hygiene. How to assist in lifting, moving, ambulation and exercise. How to prepare for the events preceding death.
Inform the Team Director, of unusual or potentially problematic patient/family issues. Attend the Hospice Team Meetings and other patient conferences as deemed necessary by the Team Director. Provide appropriate support at time of death. Ensure staff is providing 24/7 coverage to patients and families. Manage hospice aide assignments, prepares written instructions for the aide and supervises the aides. Participate in hospice orientation and ongoing education programs. Demonstrate familiarity with policies of the agency and state(s) and federal regulations pertaining to nursing services. Participate on committees and special projects and other related duties as assigned. Perform other duties as assigned including those within the scope of current, valid professional licensure/credentials (i.e. RN, SW, Chaplain, MT, HA, etc.). Registered Nurse license in state(s) practicing. At least 1 year of supervisory experience in a healthcare setting. Current Cardio Pulmonary Resuscitation (CPR) certification. Previous experience in hospice, home health, general medical surgical or patient population specific (i.e.: geriatrics, pediatric, etc.) nursing.
- CT: Minimum of one year of experience.
