Posting Intro:

Seasons Hospice & Palliative Care is seeking a Social Worker to join our team of professionals. We make a difference every day to patients and families coping with end of life illness in creating a center of excellence in hospice care.  ($2,500 Sign-On Bonus)

Overview:

The Social Worker supports the business development activities of the organization. As a member of the hospice interdisciplinary team, provide social service counseling and guidance to facilitate the maximum function and coping capacity of each patient and family member while following all hospice policies and procedures, in accordance with the interdisciplinary plan of care. Support the Provider Relations activities of the organization. Provide bereavement counseling to family members as identified.

Responsibilities:

Assist the core members of the hospice team in understanding significant social, spiritual, and emotional factors related to the patient’s health problems to establish a plan of care that fosters the personal worth, spiritual well-being, and human dignity of each patient.

Participate as a member of the interdisciplinary team and in the development and review of the plan of care for assigned patients.

Assess the social, spiritual, and emotional needs/factors in order to estimate the patient’s and involved caregiver’s capacity and potential to cope with the problems of daily living and with the terminal diagnosis and illness.

Help the patient, family, and involved caregiver to understand, accept and follow medical recommendations and provide services planned to assist in achieving the optimum social, spiritual, and health adjustment within their capacity and level of health literacy.

Prepare the patient to deal with the changes and the family to support the patient including education on Advanced Directives.

Utilize all available resources, such as family, hospice, and community agencies, to assist the patient and family to live better within the limitations of the illness.

Assess bereavement needs of patient’s family members after patient’s death and provide bereavement counseling as identified in the bereavement plan of care.

Provide discharge planning related to change of level-of-care or community placement.

Observe, record, and report social, spiritual, and emotional changes.

Document all services in the patient’s electronic medical record.

Participate on committees and perform other duties and special projects, as assigned.

Will participate in the on-call rotation and provide after-hours coverage as specified by needs of the agency.

Will provide supervisory or field instructor guidance to supportive care interns as needed.

In addition to the above duties, Social Workers working on an Inpatient Hospice Center (IPC) are expected to perform the following:

  • Ensures patient smoothly transitions from one hospice level of care to another; serves as a patient navigator and contact for patients during transition.
  • Facilitates care meetings in collaboration with hospice physician, team manager, patient, and involved care giver to ensure smooth delivery of hospice services, care coordination, advanced care planning and counseling for withdrawal of medical interventions.
  • Develops and maintains effective working relationships with care teams that generate superior clinical and business performance.
  • In addition to the above duties, Social Workers working for a Palliative Consult Service are expected to perform the following:
  • Serves as an integral member of the healthcare team providing assessments, coordination, treatment planning, information and referral to community resources and other social work services to meet the complex needs of a patients and families in the hospital, home, and post acute setting.
  • Provide counseling for disease acceptance and understanding.
  • Takes, reviews, evaluates and prioritizes written and oral referrals.
  • Maintains documentation records and data collections across all clinical records including Seasons Hospice electronic medical record and the host hospital system.
  • Serves as a liaison between patient and Seasons Hospice for maintaining positive relationship with Seasons Hospice and providing continuity of care.
  • Ensures patient smoothly transitions from one area of care to another; serves as a patient navigator and contact for patients during transition.
  • Coordinates/schedules the team interview/consult with the patient and involved care giver.
  • Develops and maintains effective working relationships with care teams that generate superior clinical and business performance.
  • Provide supervision of interns (if permitted by academic institution).
  • Additional Job Functions for a Level II Social Worker:
  • Provide supervision of interns.
  • Participate on a committee within the organization.
  • Be an active presenter on the speaker’s bureau, and support the business development activities as opportunity is available.
  • Facilitate bereavement groups or workshops as appropriate.
  •  Additional Job Functions for a Level III Social Worker:
  • Provide clinical supervision to other social workers towards higher level of licensure.
  • Provide supervision of interns.
  • Participate and chair a committee within the organization, including ethics consultations.
  • Be an active presenter on the speaker’s bureau, and support the business development activities as opportunity is available.

Qualifications:

Must have a minimum of a Master of Social Work (MSW) degree from a school of social work accredited by the Council on Social Work Education.

Qualifications for state-specific requirements above Seasons minimum education standards are as follows:

o AZ: Must be licensed by the state at a minimum of LMSW.
o CT: Must have a minimum of a Master of Social Work (MSW) and be licensed as an LCSW in the State of Connecticut; or have an MSW and be directly supervised by an LCSW on staff.
o Minimum of three years’ experience in a health care setting.
o IL: Must be licensed in the state as an LSW or LCSW.
o MA: Must be licensed to practice social work in the state as a Licensed Independent Practitioner of Clinical Social Work, or be a Licensed Certified Social Worker.
o MI: Must be licensed in the state as an LSW, LMSW, or LCSW.
o MD: Must have clinical experience in counseling and casework and be licensed by the state as an LGSW, LCSW, or LCSW-C.
o NJ: Must have an approved license by the NJ Board of Social Work Examiners.
o OR: Must have CSWA.
o TX: Must be licensed to practice social work in the state.
o WI: Must be certified/licensed in the state

Minimum one year of social work experience in a healthcare delivery system is required; two years’ experience is preferred.
Must have reliable transportation, a valid driver’s license, and the minimum state required liability auto insurance.
Demonstrated commitment to the Seasons Hospice philosophy of care, values, mission statement and organizational culture.

Additional Qualifications for a Level II Social Worker:

Must hold the highest level of licensure within the state, as indicated in the table below; and/or currently hold the ACHP-SW advanced certification.
Must have greater than two years of full-time experience as a social worker.

Additional Qualifications for a Level III Social Worker:

Must hold the highest level of licensure within the state, as indicated in the table below.

Must hold the ACHP-SW advanced certification.

  •  WI Only – Must hold the highest level of licensure within the state and/or the ACHP-SW advanced certification.

Must have greater than five years of full-time experience as a social worke
