The Patient Care Coordinator is responsible for the work flow coordination within Home Care Home Base. In addition, PCC must be able to do an assessment and evaluation of patients. The Patient Care Coordinator assures compliance with agency standards and state and federal laws.

4350 - ProMedica Home Health Care - Sylvania, Ohio

Minimum of one year experience in providing medical, oncology, home health care, or hospice nursing preferred, may be specified by state. Previous management or supervisory experience preferred.


EducationLevel : Currently licensed as a Registered Nurse (RN) in good standing with the State Board in which he/she practices.
