Job Summary
Job Description :
They are responsible for providing basic nursing care to patients who require assistance with their daily activities but are not in need of hospitalization. Home Health LPN/LVNs work under the direction of a registered nurse or physician and collaborate with other healthcare professionals to develop and implement a patient's care plan. They may perform tasks such as administering medications, dressing wounds, monitoring vital signs, and assisting with personal hygiene. They also provide education and support to patients and their families on managing chronic conditions and maintaining overall health and wellness. Home Health LPN/LVNs must be knowledgeable about nursing practices and be able to work independently while adhering to healthcare regulations and policies.
Job Requirements :
- A Home Health LPN/LVN must have a valid and active license to practice as a licensed practical nurse or licensed vocational nurse in their state of practice. To obtain licensure, candidates must pass the National Council Licensure Examination (NCLEX-PN).2 Years of experience
