Hospital readmissions are an important measure of quality care in U.S. hospitals. Among the likely reasons for readmission are shortfalls in how organizations execute transitions of care, resulting in inadequate communication, inadequate follow-up care, and medical errors. Each can be prevented by implementing effective transitions. This course will review how your organization can effectively handle transitions of care.

The goal of this course is to provide nurses in acute care with the best practices for transitioning care.


After taking this course, you should be able to:

  • Describe the effect of transitions of care on older adults.
  • Describe the importance of medication reconciliation as related to transitions of care.
  • Identify at least two ways to improve transitions of care.
