Goal and Learning Outcomes

Approximately 1 in 5 Medicare beneficiaries discharged from the hospital is readmitted within 30 days at a cost of $26 billion per year (CMS, 2017). Many of these readmissions may be preventable, representing an estimated $12 billion in Medicare spending (CMS, 2017). Readmissions negatively affect patient outcomes, waste taxpayer dollars, and increase healthcare system financial burdens. While this is not a new problem, the Affordable Care Act has provisions targeted at decreasing hospital readmission rates. One focus area is the transition of care between facilities or providers, or “care transitions.” This course presents concepts of “transitions of care” (or “care transitions”) to improve patient outcomes and decrease avoidable readmissions.
