Goals and Learning Outcomes

Coordination of services throughout the length of stay, from admission through death and bereavement, is more highly regulated today than it ever has been. Scrutiny by the Centers for Medicare and Medicaid Services finds gaps in care planning, coordinated management of services, and documentation. Hospices must make the shift for better managed and coordinated patient care within the case management model. In this course, you will learn about the case management model and the role of the case manager.


After taking this course, you should be able to:

  • Explain the advantages of the Case Management model of care.
  • Discuss the importance of Case Management as related to on-call services.
  • Describe the regulations governing the hospices QAPI program.
  • Explain the role of Case Managers in patient outcomes and QAPI.
