I read the article Dream teams (Nurse.com/Article/Dream-Teams), about ambulatory care and smiled. Ambulatory nurses do great work across the continuum. It really does take a village. Nurses are the translators for physicians and patients/families and understand well how chronic disease progresses. Nurses focus on teaching self-management skills and how to cope with health changes.
