Sound Shore diabetes program extends national certification for 4 years

Sound Shore Medical Center of Westchester, New Rochelle, N.Y., recently was recognized again by the American Diabetes Association for its diabetes self-management education, a four-year recognition it has received since 1995.

The ADA grants recognition based on a facility meeting the national standards for diabetes self-management education. Recognition benefits include access to ADA staff and information.

With the support of Sound Shore’s healthcare team and increased knowledge and awareness of diabetes, patients assume a major part of the responsibility for their diabetes management, according to a medical center news release. A reduction in hospital admissions and some of the acute and chronic complications of diabetes may be affected by self-management education. In addition to self management education, the program staff provides screening through community outreach. They participate in health fairs, sponsor a support group called Sweet Talk and offer educational seminars, consultation and resources to SSMC’s nursing staff.

The program represents the cornerstone of the newly opened Jacee Lynn Cappelli Diabetes Center, featuring the Elaine and Melvin Merians Diabetes Education and Conference Center. •
