Case Management is the process of

multiple settings. There are many models for case
management given client, context, or setting.
Case management is directed toward a targeted or
selected client/family population such as
transplant, head-injured, or frail elderly
clients. The goals are to center services around
the patient, to foster patient self-managed care,
and maximize efficient and cost-effective use of
health resources. The focuses are cost-saving and
continuity and quality of patient care. The Nurse
Case Manager utilizes clinical pathways in
assessment and monitoring of clients and
healthcare delivery |
