Job Description
The MDS Coordinator (LVN) is responsible in the development, completion and transmission of assessments in accordance with the regulatory requirements and reviews and manages reimbursement. Ensures assessments accurately reflect the physical, mental and psychosocial status of each patient; ensures appropriate documentation to report and support services provided and assessment accuracy. The MDS Coordinator (LVN) is responsible to case manage care plans, including rehabilitative programs. It is important that this individual demonstrate knowledge of clinical standards of practice, regulations and reimbursement governing long-term care and Medicare participation. In addition to these tasks, an MDS coordinator performs a broad range of clinical nursing duties and is often also responsible for ensuring compliance with other regulatory agencies.


Education: Graduate of an accredited School of Professional Nursing.

Experience: One (1) year of skilled nursing facility, long term care experience, rehabilitation nursing or other healthcare related facility and previous experience in completing Minimum Data Set (MDS), Resident Assessment Instrument (RAI) process and/or Care Plans. Experience with Medicare/Medicaid reimbursement, clinical resource utilization and/or case management is highly desirable.

  • Must be a LPN/LVN in good standing and currently licensed by the State of Texas;
  • Certified in Basic Life Support
  • MDS Certification - American Association of Nurse Assessment Coordinators (AANAC) preferred.

Working knowledge of the RAI process and of clinical reimbursement as related to the RAI process.

Must be knowledgeable of general, rehabilitative and restorative nursing and medical practices and procedures and laws, regulations and guidelines governing long-term care and rehabilitative services.

Must have ability to perform complex and diverse administrative duties that involve application of procedures, independent analysis, interpretation of data and demonstrated appropriate judgment.

Able to perform advanced computer skills, data entry into CMS required software.   • Participate in peer support / evaluation, in services, education programs, and staff meetings, Quality Improvement activities as assigned; and desired client-centered outcomes. Reviews the state validation reports and ensures that appropriate follow-up action is taken.
  • Creates an opportunity for family participation in the care planning process.
  • Assists in the development of a comprehensive resident assessment and care plan.
  • Coordinate the development and completion of the resident assessment (MDS) in accordance with all Federal and State guidelines.
  • Selects the correct (ARD) Assessment reference date (ARD) and correct reasons for assessment that capture all resources with coordination with therapy department to ensure appropriate payment by Medicare/Medicate systems
  • Accepts overall responsibility for the Resident Assessment Instrument and care plans for all SNF patients.
  • Keeps abreast of current federal and state regulations, as well as professional standards and makes recommendations for changes in policies and procedures to the manager.
  • Participates in continuing education related to MDS every year.
  • Ensures accurate patient clinical assessments that support appropriate clinical interventions.
  • Inputs information accurately into MDS record and transmits to Medicare within the required time frames.
  • Coordinates team participation and input in the identification and development of measurable long-term and short-term goals and barriers toward successful discharges.
  • Assures continuity of information including assessment MDS, RUGs IV and care plans through audits.
  • Responds to referrals in a timely manner and performs screenings with skill and accuracy.
  • Coordinates the admission process.
  • Attends presentations, reviews information and shares it with staff as it relates to updates and new requirements/aspects of the patient assessment, goal planning and documentation process.
  • Functions as a resource to other members of the treatment team.
  • Ensures safe care to patients, staff and visitors; adheres to all Memorial Hermann policies, procedures, and standards within budgetary specifications including time management, supply management, productivity and quality of service.
  • Promotes individual professional growth and development by meeting requirements for mandatory/continuing education and skills competency; supports department-based goals which contribute to the success of the organization; serves as preceptor, mentor and resource to less experienced staff.
