Position Summary:

We have an exciting opportunity to join our team on a temporary basis as a Nurse Practitioner with the Geriatric Consult Team.

In this role, the successful candidate provides nursing leadership for the Geriatric Program in the development of geriatric-related patient care standards, initiation and implementation of new educational programming, and expansion of geriatric services consistent with the mission and goals of NYUHC. Supports the leadership efforts of the Geriatric Program in collaboration with the Medical Director. Establishes priorities and implements long and short-term action plans with the Geriatric Consult Team. Participates in care coordination of geriatric patients across care settings. Collaborates with interdisciplinary team in discharge planning including identifying appropriate community resources to achieve optimum outcomes. Participates in geriatric research and development. Provides direct clinical care as Nurse Practitioner in the management of geriatric patients

Job Responsibilities:
  • Assesses patients referred to the Geriatric Team at NYUHC for geriatric focused services including geriatric syndrome management, functional maintenance/recovery, chronic co-morbid condition management, and extended care options.Performs directed health history, physical examination and psychosocial assessment on Geriatric Team patients.
  • Works collaboratively with the Geriatric Team, attending physicians, house staff, consulting physician(s), and other disciplines to identify, develop, implement, create and modify as necessary an appropriate plan of care that considers potential outcomes and goals of care, maximizes patient/family preference, and that enhances quality, access, and clinical outcomes.
  • Prescribes medications for geriatric care patients according to NYS law, including controlled substances with DEA authorization. Orders laboratory, radiological and diagnostic studies as indicated.Prescribes medical equipment, devices, physical and occupational therapy, and home health services per practice protocol.
  • Evaluates patient responses to treatment and other therapeutic interventionsSupports service and unit level leadership by providing consultative guidance on the care of the elderly.
  • Oversees appropriate placement of geriatric patients upon discharge.Functions within scope of practice and delineation of privileges as Geriatric Nurse Practitioner.
  • Practices as a member of the allied health staff as outlined in the by-laws, rules and regulations of the Medical StaffActs as a professional role model to all levels of staff.
  • Articulates and demonstrates nursing process when exercising responsibilities and in providing support and guidance to staff.Develops, implements, and utilizes evidence-based standards in geriatric care.
  • Considers needs and behaviors of specific patient age and cultural groups in all patient care activities.Uses evidence in daily practice through implementation of established guidelines and standards of care.
  • "Supports the mission, philosophy, standards, goals and objectives of the institution, Departments of Nursing, Medicine, and Geriatric Service.Establishes and implements program goals in collaboration with the Medical Director, and the Geriatric Team."
  • "Jointly develops, implements, and maintains geriatric program related standards with the Geriatric Team, and Nursing Practice Councils.Coordinates the Geriatric Program, in collaboration with the Medical Director, the Geriatric Consultation Team and Director of Advanced Practice Nurses & Physician Assistants."
  • Ensures seamless transition of care across settings through collaborative communication with care providers.
  • Develops criteria for Geriatric Team referral with the Medical Director.Assists in the development and implementation of a viable fund-raising plan with the Office of Development.
  • Assists in the development of a marketing strategy and public information plan with the Office of Public Affairs, and the Office of Strategic Business Development and Planning.
  • Conducts activities of the geriatric program in a fiscally sound and effective manner.In collaboration with the Medical Director, ensures Geriatric Program compliance with all regulatory standards, including TJC, NYSDOH, CMS, and other related regulatory agencies. Observes NYUHC compliance policies.
  • Participates in hospital committees and represents the hospital in outside activities.
  • Promotes own professional growth and development in clinical and managerial role.Develops and implements a professional development plan.
  • Maintains current expertise in medicine and geriatrics nursing.Contributes to the development of nursing knowledge in area of practice through publication, research, and formal/informal presentations.
  • Serves as a clinical expert and resource for the education of peers and other health professionals related to the care and special needs of the elderly.
  • Educates patient and family to facilitate and promote healthy adjustment to change(s) in health conditions.Participates in the Patient /Family Education Council.
  • Assists in the provision of interdisciplinary geriatric educational programs to educate staff at all levels.Assists in the development and delivery of community education regarding advanced care planning and related topics.
  • "Participates in the design, implementation, and evaluation of geriatric quality improvement activities and research protocols."
  • "Initiates and/or participates in relevant research activities Participates in organizations safety and quality initiatives as well as annual selection and evaluation of indicators relevant to geriatric care."
  • Evaluates success of the program through established goals and quality metrics.
  • Participates in developing, obtaining, and executing grant funded research projects.
  • "Recommends practice changes based on current literature, research, and evidence-based findings."

Minimum Qualifications:

To qualify you must have

Licensure: Current registration as a Nurse Practitioner and Professional Registered Nurse in NY State; Education: Master's Degree in nursing, advanced nursing practice in geriatrics required. Certifications: Board certification as Nurse Practitioner in Geriatrics; BLS and ACLS required; Experience: Minimum of five years of clinical practice, preferably as GNP in acute care; three or more years of progressive management or nursing leadership experience including case management. Competencies: All core competencies as outlined by The National Organization of Nurse Practitioner Faculties (NONPF, April 2011 and amended 2012); Adult-Gerontology Acute Care Nurse Practitioner Competencies outlined in American Association of Colleges of Nursing 9AACN, February 2012). Evidence of ability to manage data and use of computer applications.
