Registered Nurse Care Manager (PACT)
Duties include but are not limited to the following:
-Utilizes the nursing process and evidence-based practice to collaborate with the PACT.
-Practices specialized skills in patient assessment via telephonic and direct contact with patient in order to facilitate care transition internally as well as externally to meet the patients' need.
-Provides patient and family health education with a focus on self-management, prevention, and wellness, based on the patient's total health care goals.
-Performs other directly related clinical and administrative duties as assigned.
Work Schedule: Monday-Friday, Day Shift staggered between 0630 and 1630; Small amount of overtime to include rotating as Late Nurse and Saturday and Holiday Clinics.

Graduation from School of Nursing
Graduate of a school of professional nursing approved by the appropriate State agency, and accredited by one of the following accrediting bodies at the time the program was completed by the applicant. [NOTE: See exception in subparagraph (b) below for candidates who are enrolled in a MSN Bridge Program.] (1)The National League for Nursing Accrediting Commission (NLNAC). OR (2) The Commission on Collegiate Nursing Education (CCNE).
[The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing.
In cases of graduates of foreign schools of professional nursing, possession of current, full, active, and unrestricted registration (see paragraph 2a(3)) will meet the requirement of graduation from an approved school of professional nursing.
Condition of Employment. A registered nurse (RN) will have a current, full, active and unrestricted registration as a graduate professional nurse in a State, Territory, or Commonwealth (i.e., Puerto Rico) of the U.S. or in the District of Columbia. The RN must maintain a current, full, active and unrestricted registration to continue employment with VA.
Physical Standards. Per VA Handbook 5019, Part IV, Appendix A, all full- and part-time employees, Volunteers and trainees are required to complete a Tuberculin Test prior to employment or assignment to duties where they may be exposed to tuberculosis.
Physical Functional Requirements. Heavy lifting, 50 pounds and over; Moderate carrying, 15-44 pounds; Straight pulling (3 hours); Pulling hand over hand (3 hours); Pushing (3 hours); Reaching above shoulder; Use of fingers; Both hands required; Walking (6 hours); Standing (6 hours); Kneeling (3 hours); Repeated bending (3 hours); Climbing, legs only (3 hours); Climbing, use of legs and arms; Both legs required; Ability for rapid mental and muscular coordination simultaneously; Far vision correctable in one eye to 20/20 and to 20/40 in the other; Both eyes required; Depth perception; Ability to distinguish basic colors; Hearing (aid permitted).
English Language Proficiency. RNs appointed to direct patient care positions must be proficient in spoken and written English as required by 38 U.S.C. 7402(d) and 7407(d).

Applicant's education and length of nursing practice (experience) will be considered by a Nurse Professional Standards Board in determining the grade and salary of the applicant selected. Experience as a RN will be evaluated to determine if the experience is of an acceptable level of quality with regard to the following four dimensions of nursing: Practice, Professional Development, Collaboration and Scientific Inquiry. Each dimension of practice has criteria that demonstrate essential core competencies for VHA Nursing Personnel. The criteria are as follows: Practice: practice, ethics and resource utilization, Professional Development: performance, education/career development, Collaboration; collaboration, collegiality, Scientific Inquiry: quality of care, research.
The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.
Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience.
Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree in nursing or related field and approximately 2-3 year's of nursing practice/experience.
Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religions; spiritual; community; student; social). Volunteer work helps build critical competencies, knowledge, and skills and can provide valuable training and experience that translates directly to paid employment. You will receive credit for all qualifying experience, including volunteer experience.
BLS/ACLS Requirements: Current certification in BLS is required for all staff providing direct patient care, and police. Additionally, current ACLS is required for patient care staff that work in specified or high risk areas (SICU/ICU/CCU/MICU/PACU/ED/and all procedure suites using moderate sedation, etc.). If you are selected for a position which requires BLS/ACLS certification, you will be required to have proof of certification prior to being boarded for appointment with VA. If selected, obtain such certification in order to avoid any delays in the hiring process.
-Current Basic Life Support (BLS) certification is required.
