Providence is calling a Clinical Manager, Home Health Services. The Home Health Clinical Manager is responsible for oversight and supervision of the daily clinical operations of the ministry’s home health program. Supervises and mentors field practice. Directs plans and coordinates activities of staff rendering patient care. Assures the services are appropriately scheduled and provided according to clinical needs, staffing ability and competency. The recruitment, interviewing, hiring, orientation and oversight of skills competency and performance as per state and/or federal regulations
Providing support and consultative services to the nursing, therapy, social work and home health aide staff
Assuring that assessments and care plans are established at the start of services, arranging ongoing monitoring of performance and competency as per state and federal regulations
Providing supervision, ongoing skill development and education opportunities
Monitoring and follow-up of personnel requirements and assuring maintenance of complete and accurate files and documents.
Facilitating implementation of a cost effective plan for delivery of home services with the goal of optimal patient outcomes
We're seeking an individual who performs all duties in a manner that promotes teamwork and reflects the Mission and Philosophy of our founders, the Sisters of Providence.
Position Specifics. This position is located at Providence SoundHomeCare &Hospice in Olympia, WA. This is a full-time.
REQUIREMENTS Education:Bachelor Degree preferred. Graduate from an accredited school required. Licenses/Certifications:Must hold a license in the healthcare field as an RN, PT, or SLP; must have current CPR certification. Certification as OASIS Specialist preferred, or obtained within 6 months of employment. Must have current Washington State driver’s license required. And daily access of an insured, road worthy, reliable vehicle and current auto insurance Experience:Minimum of five (5) years experience in an acute care facility, two (2) years home health experience, and two (2) years management experience required. Recent clinical case management experience in home health strongly preferred.
